p.1
The provision is inadequate
n The provider has failed to meet all actions that were set at previous inspections. She has failed to rectify weaknesses in staff's practice, behaviour management and risk assessments. Staff are not effectively deployed to support children, which hinders children's learning and development. n Children are not safe. The provider does not carry out safe staff recruitment. She does not vet staff appropriately, for example, she does not carry out reference checks for family members and staff do not have appropriate Disclosure and Barring Service checks in place. Staff also have a weak understanding of signs and symptoms of child protection and the 'Prevent' duty. This compromises the children's welfare. n The provider and staff are unable to highlight risks in the nursery. Staff fail to follow their own hygiene practices and do not consistently teach it to the children. This further puts children's welfare at risk.
p.1
The provision is inadequate
n Children's behaviour is poor and disruptive. Staff do not provide enough challenge for children and they quickly become bored. In addition, staff are not able to explain to children why they need to stop certain behaviours. This hinders children's self- confidence, and social and emotional development. n Children's learning is greatly hinder by staff's lack of knowledge and understanding of the early years foundation stage. Staff fail to plan activities to support their key children's learning and development, and are unable to support other staff's key children. They are also unable to track accurately any progress children are making.

p.2
Effectiveness of leadership and management is inadequate
Safeguarding is not effective. The provider has taken action to remove the mould identified as a health risk at the last inspection. However, the provider has failed to address all the actions set at the last inspection. She lacks the ability and understanding to address these weaknesses. In addition, the provider lacks the understanding of safe staff recruitment. She employs family members and does not have references in place for them. Staff who have worked at the nursery for a long period do not have the appropriate Disclosure and Barring Service checks in place. Staff also lack an understanding of safeguarding procedures to ensure children's safety. Staff are not aware of the 'Prevent' duty or the correct procedures to follow if they have concerns regarding a child's welfare. This puts the children at significant risk. Although the provider works with the local authority, this is not effective. The provider has not met the

actions the local authority has raised in its reports to assist with improving her and her staff's practice in the nursery.
p.4
Quality of teaching, learning and assessment is inadequate
The quality of teaching is poor. Staff do not provide any challenging activities to reflect their current abilities fully. Tracking of children's learning is not accurate and shows, in some instances, children are regressing with their learning. This has not been highlighted by the provider and therefore action has not been sought as to why this has happened. Staff lack an understanding of children's stages of learning, to progress them towards the early learning goals. For example, staff state they will use the alphabet song to teach children 'ABC'. Staff do not teach children how to link the letters they sing to the letter sounds, and they also lack the understanding of how singing a song can help to support children's learning. Staff also have an inadequate understanding of activities to carry out to support children's learning. For instance, staff put out pens and paper as they feel children need to 'learn how to cut paper'. They are not able to explain how this

supports children's learning and do not provide scissors for the children to use in order to cut the paper. Staff are not able to support their own key children and those of other staff when they are not present. They are unaware of children's next steps of learning and fail to use children's interests as they do not know the children well enough to support this. This is detrimental to children's emotional well-being. Children's developmental reports are not an accurate reflection of their learning. This hinders teachers' abilities to assess what stages children have reached when they move to school. This provides poor continuity of care.
p.4
Personal development, behaviour and welfare are inadequate
Staff are not able to keep children safe. Risk assessments are ineffective. The provider sets up water activities underneath plug sockets. In addition, they place tables underneath open windows. Although they have safety catches on the windows, they still open wide enough for children to fit through. Bleach is also easily accessible in the children's bathroom. During breakfast routines, staff teach children to hold fruit in their hands as they cut it up. They do not teach them the safe way to use knives. All of this puts children at significant risk of harm. Staff do not nurture children's self-esteem. For example, during the inspection, staff tell children in the garden to stop kicking the ball or they will take it away from them. During lunchtimes, staff tell children they need to eat all of their lunch or they will not be given pudding. This is detrimental to the children's learning and does not teach them differences between right and wrong. Staff fail to

follow appropriate hygiene procedures when handling and preparing food. They do not wash their hands before they prepare snacks for the children. In addition, they do not consistently teach the children the importance of washing their hands before meals. This does not protect children from cross-infection and potential contamination of food.
p.5
Outcomes for children are inadequate
Children are not prepared for the next stage of their learning and subsequent move to school. Children do not make the progress of which they are capable. Children do not have access to activities that help to support their learning and development. During the inspection, as children play with the water, they are continually told to stop moving water into different containers. This hinders their exploration and learning, and they are not being sufficiently challenged. Children also have resources taken away from them, which prevents them learning further.
