p.1

provider

p.1

provider

The registered service manager, who is also the nominated individual, registered with Ofsted in May 2024.

p.1

This monitoring visit

A monitoring visit took place on 16 July 2026 due to Ofsted receiving notifications about serious incidents involving children. The inspectors found there were significant concerns about children’s safety and the leadership and management of the undertaking. Following this visit, 2 compliance notices were issued under regulation 4 and regulation 6. In addition, a notice restricting the accommodation was issued. This notice prevented any more children from being accepted into the supported accommodation.

p.1

This monitoring visit

The purpose of this monitoring visit was to review the action the provider had taken to address the shortfalls highlighted in the compliance notices and review the restriction on the accommodation. The provider has failed to take sufficient action to meet the requirements of the compliance notice issued under regulation 4. This has been reissued. The provider has complied with the restriction of accommodation notice and this remains in place until 8 October 2026. Inspection report for supported accommodation provider - monitoring visit: Sanctuary Ltd Support Services The provider has implemented a new referral and compatibility assessment form to help them consider the needs of children referred to the service. This provides a framework to consider each child’s history as well as oversight of their progress, significant incidents, risk and support plans, and safeguarding concerns. This means the provider has a framework to consider children’s co-existent risks and needs. The

This monitoring visit

provider has undertaken an in-depth review of previous children they have supported and shared lessons learned with the staffing team to support practice development.

p.2

This monitoring visit

Leaders have also completed a review of learning from safeguarding incidents and children’s poor endings in the supported accommodation. Leaders are developing a better understanding of the strengths and weaknesses of their service. However, this learning is not yet embedded in practice.

p.2

This monitoring visit

The provider has implemented improved monitoring systems to assist them in maintaining the health and safety of children they support. There are improvements in children’s environments, maintenance and repairs have been completed and hazards have been removed. Children’s accommodation is now clean and tidy. This encourages children to develop independence skills regarding maintaining their environment that they need as they move towards adulthood.

p.2

This monitoring visit

The registered service manager has updated the children’s guide, to provide children with relevant information about what they are entitled to receive from the service. However, they have not clearly communicated these rights to children. Furthermore, staff have not ensured that children understand their rights and entitlements in a way that supports them to develop the knowledge and confidence to advocate for themselves independently in the future.

p.2

This monitoring visit

The registered service manager has identified that staff require additional training so they can better support children who may be at risk of knife crime or child criminal exploitation. They have sourced external facilitators, so all staff can receive the relevant face-to-face training to help children, but this has not yet taken place.
