p.1

This service is owned and managed by a private provider. It offers accommodation for up to 2 children in one shared group-living home. The home is not limited to accommodating looked-after children and care leavers.
p.1
provider
No children were living in the home at the time of this visit.
p.1
provider
The service manager, who is also the nominated individual and sole director, registered with Ofsted when the service registered in March 2025.
p.1
This monitoring visit
Ofsted carried out a monitoring visit on 4 February 2026 in response to concerns raised regarding the provider’s failure to notify Ofsted of all serious events, and the suitability of children placed in the supported accommodation. Following that visit, Ofsted issued 2 compliance notices relating to the notification of serious incidents and the referral and admission process. A notice restricting accommodation, in force until 30 April 2026, was also issued.
p.1
This monitoring visit
Ofsted carried out a further monitoring visit on 31 March 2026 to monitor the provider’s progress towards meeting the steps outlined in the compliance notices and to ensure compliance with the notice restricting accommodation. Sufficient evidence was found that the provider had met the requirements of the compliance notices. However, concerns remained regarding the maintenance and safety of the accommodation.
p.1
This monitoring visit
On 28 April 2026, Ofsted carried out a further monitoring visit to review the status of the notice restricting accommodation. Concerns remained about the safety and Inspection report for supported accommodation provider - monitoring visit: Universal Healthy Minds Limited quality of the accommodation. Following a case review, a further notice restricting accommodation was served to prevent the provider from admitting any children.
p.1
This monitoring visit
The purpose of this visit was to review compliance with the notice restricting accommodation in force from 1 May 2026 to 23 July 2026 inclusively. The inspector also considered the provider’s submission of written representations to appeal the notice of proposal to cancel the registration of this service, which Ofsted dated 14 May 2026. The registered service manager was present and facilitated the visit. They confirmed that no children had been admitted to the service since the last child moved out in March 2026.
p.2
This monitoring visit
At the time of the visit, no effective management systems had been established and embedded. The provider had informed Ofsted that competent management oversight was available 7 days a week following the appointment of a new management team. It transpired that the appointed staff would not start their roles until 3 August 2026. It remains a concern that the lack of management and staff would present a risk to children moving into the home.
p.2
This monitoring visit
Leaders and managers have failed to provide the full recruitment files for the new management team. One application that was made available included references that did not correlate with the applicant’s employment history. The question about the applicant’s suitability to work with vulnerable children and adults was not made explicitly clear in the employment references. These gaps in practice do not provide sufficient assurances that leaders and managers have safely assessed the management team’s competence for the roles.
p.2
This monitoring visit
Leaders and managers have failed to demonstrate the delivery of refresher training to all staff. Furthermore, some of the training identified by the provider in their written representations is not included in the current training matrix. It is therefore unclear whether all the required training has been delivered and refreshed before staff start supporting children. Leaders and managers have failed to ensure that the individuals identified as designated safeguarding leads have received the relevant training. The absence of this important training places children at risk of harm.
p.2
This monitoring visit
There are ongoing concerns relating to the quality and safety of the accommodation. For example, the front door of the property still has several bolts embedded, and the manager did not know why they are still in place. The fence at the back of the property still shows signs of damage. Non-working lights in the kitchen have still not been replaced since previous monitoring visits. One of the sofas in the lounge has a noticeable tear and damage that had not been identified or addressed for repair promptly. It is of further concern that the same sofa was included in the provider’s written representation’s submission several weeks ago as an exemplar for improvement. This reinforces concerns about the weak management oversight of the quality assurance processes.
p.2
This monitoring visit
The current fire safety arrangements at the property do not support the safe evacuation of children, staff and visitors in the event of a fire. The fire evacuation Inspection report for supported accommodation provider - monitoring visit: Universal Healthy Minds Limited arrangements are not confirmed in the reviewed fire risk assessment. The back of the property is shared with a trading estate that has large entry and exit gates that may be secured at night, which also encloses the back of the property. Leaders and managers have failed to assess this potential fire risk and put a contingency plan in place. The poor management oversight of the fire safety arrangements places children, staff and visitors at risk of significant harm.
p.2
This monitoring visit
The provider has failed to assure Ofsted that children would be safe if they were admitted to this service. Ofsted has issued a further notice restricting accommodation to ensure that no children are admitted to the service.
