Inspection report for supported accommodation provider - monitoring visit: Eleven D's Ltd 1 Eleven D's Ltd Registered provider: Eleven D's Ltd
p.1
Inspected under the social care common inspection framework
p.1
provider
Eleven D's Ltd registered with Ofsted on 9 January 2025 to provide non-ring-fenced supported accommodation. At the time of registration, the provider had one property, offering supported accommodation for up to 4 children. At the time of this visit, the provider had 6 properties, offering supported accommodation for up to 44 children.
p.1
provider
This visit was unannounced. On the day of this visit, 9 children were receiving support from this provider.
p.1
provider

p.1
This monitoring visit
The registered person has failed to operate the service in line with their statement of purpose and has therefore failed to prioritise children's needs and wellbeing.
p.1
This monitoring visit
During the monitoring visit, Ofsted found significant and widespread failings relating to the leadership and management of the service because their overall oversight of the service operation is weak. Leaders and managers have failed to ensure that monitoring systems are effective, which has meant that shortfalls in practice have not been recognised and promptly acted upon.
p.1
This monitoring visit
Staff do not take proactive steps to address any children's emerging behaviours of concern, such as child exploitation. Children's risk assessment do not reflect known or developing concerns, leaving children at risk of harm. The result is that risk Inspection report for supported accommodation provider - monitoring visit: Eleven D's Ltd 2 management strategies are ineffective. It is not clear to what extent staff offer children planned, targeted support to help minimise any risks because there are no sufficient records to show this.
p.1
This monitoring visit
Staff were unable to access all relevant systems in relation to information about children. This means that staff are supporting children without knowing their vulnerabilities and needs. Children do not have individual support plans, and some staff are unclear about the focus of the support that children receive. Children's needs are not being met because the staff do not know what the children's needs are. There is limited written evidence to demonstrate that children receive the personalised guidance they need to progress towards independence and living independently.
p.2
This monitoring visit
Since registration, the registered service manager or nominated individual have failed to notify Ofsted of several serious incidents, including allegations against staff, local authority designated officer involvement, police callouts and when children have been physical harmed. This shortfall means that Ofsted does not know how the provider responds to protect children from harm.

The provider has failed to provide their quality of support reviews to Ofsted in a timely manner, and the content of reports does not meet regulatory requirements.
