provider

At the time of the inspection, the service was providing supported accommodation for 4 children aged 16 and 17.

p.1

This monitoring visit

The purpose of this monitoring visit was to investigate information received that suggested there may be a breach of the regulations.

p.1

This monitoring visit

The registered person has not maintained effective oversight to ensure that the accommodation is maintained to an appropriate standard. Leaders have not implemented an effective system to record, monitor and oversee maintenance requirements to ensure that identified repairs are completed in a timely manner. As a result, children live in an environment that does not consistently meet expected standards of safety, repair and quality.

p.1

This monitoring visit

Effective admission processes have not been implemented to assess the suitability of children moving into the accommodation. The registered person has not effectively considered children’s individual needs and vulnerabilities and the potential impact on other children living in the accommodation before they move in. Missed opportunities were identified to obtain and consider relevant information from a placing authority to inform this decision-making. For one child, the unsuitable Inspection report for supported accommodation provider - monitoring visit: PSM Healthcare Limited 2 admission of another child directly contributed to an increase in their presenting vulnerability and exposure to risk.

p.1

This monitoring visit

The registered person has not consistently notified the regulator of serious incidents involving children. This has limited effective external scrutiny of significant incidents and the actions taken by staff and leaders to safeguard children. Despite this being raised at the last full inspection, the registered person has failed to take effective action to address this shortfall.

p.2

This monitoring visit

Systems for recording serious incidents are ineffective. They do not ensure that records are accurate, sufficiently detailed, or clearly evidence the actions taken by staff to manage and reduce risks to children. Management oversight of safeguarding records is also ineffective, resulting in poor practice not being identified, which prevents staff from receiving adequate training to strengthen the quality of practice.

p.2

This monitoring visit

Leaders have not ensured that staff receive regular supervision in accordance with the organisation’s policy. This limits opportunities for staff to reflect on their practice and to identify learning and development needs. As a result, leaders cannot be assured that staff have the skills and experience needed to provide safe and effective support for children.

p.2

This monitoring visit

Children spoke positively about their relationships with staff and said they feel supported. Feedback from children’s placing authorities was also positive, highlighting effective communication from staff and managers and the progress that children have made.

p.2

This monitoring visit

Following feedback provided on inspection, leaders responded proactively and took prompt action to begin addressing the shortfalls identified. Inspection report for supported accommodation provider - monitoring visit: PSM Healthcare Limited 3
