Inspection report for children’s home - monitoring visit: 2872012 1 Registered provider: Beacon Haven Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

The home is owned by a private provider and offers care for up to 4 children who experience social and emotional difficulties.

p.1

Information about this children’s home

The home registered with Ofsted in January 2026. In March 2026, the registered manager requested to cancel their registration. As a result, the full inspection was brought forward. Following the inspection on 1 and 2 April 2026, the home was judged inadequate. Both children who were living at the home at the time of the full inspection have been supported to move out.

p.1

Information about this children’s home

This is the home’s third monitoring visit since the full inspection. A new manager was in post during the full inspection and the first 2 monitoring visits, and they had applied to register with Ofsted. However, since the second monitoring visit, they have withdrawn their application to register. Another new manager has recently been appointed and is in the process of applying to register with Ofsted.

p.1

Information about this children’s home

No children were living in the home at the time of this visit.

p.1

This monitoring visit

During the full inspection, serious and widespread failures in safeguarding practices, ineffective leadership and management and poor care and experiences of children were identified. The home was judged inadequate, and 2 compliance notices were issued under Regulation 12 (the protection of children standard) and Regulation 13 (the leadership and management standard) of The Children’s Homes (England) Regulations 2015. In addition, a notice restricting accommodation was served to prevent any further admissions to the home.

p.1

This monitoring visit

On 20 April 2026, a monitoring visit was carried out to review the effectiveness of the actions taken by leaders and managers to address the shortfalls identified in the Inspection report for children’s home - monitoring visit: 2872012 2 compliance notice issued under Regulation 12. It was determined that although leaders and managers had taken some action to address the shortfalls identified in the compliance notice, they were not yet able to demonstrate effective oversight of safeguarding concerns or a clear understanding of safeguarding procedures. As a result, the compliance notice was reissued, and the notice restricting accommodation remained in place.

p.1

This monitoring visit

On 18 May 2026, a second monitoring visit was carried out to review the actions taken by leaders and managers to address the shortfalls identified in the compliance notices issued under Regulations 12 and 13. It was determined that sufficient action had been taken to meet both compliance notices. All requirements and the recommendation were restated, and the notice restricting accommodation remained in place.

p.2

This monitoring visit

The purpose of this visit was to review whether sufficient action has been taken to allow the notice restricting accommodation to expire on 26 June 2026.

p.2

This monitoring visit

Leaders and managers have implemented a robust improvement plan to address the shortfalls identified by Ofsted. This includes additional staff training, reflective practice sessions and new systems to support recording, monitoring and oversight of the quality of care for children.

p.2

This monitoring visit

Leadership and management arrangements have been strengthened, and staff are being helped to better understand the home’s approach to care and support. Staff speak positively about the increased support that they have received, and they recognise the improvements that have been made. Staff demonstrate a good understanding of their new ways of working to meet children’s needs and keep them safe.

p.2

This monitoring visit

Following the last monitoring visit, there were 4 open concerns about staff practice and conduct. Three of these concerns have now been closed, with actions and outcomes being shared with the relevant professionals. One concern remains open. An additional historic concern has been raised about another member of staff. Leaders and managers have responded appropriately, and the investigation process is ongoing.

p.2

This monitoring visit

Leaders and managers have strengthened their assessments of the ability of staff to meet children’s needs and keep them safe before they move into the home. Leaders and managers are clear that they will ensure that the new admission procedures are followed stringently and that children experience gradual, safe and positive admissions to the home.

p.2

This monitoring visit

Although leaders and managers have taken action to address the vulnerabilities that were identified by Ofsted, many of the improvements are in their infancy and, as there are currently no children living at the home, the impact of the changes cannot be fully evaluated. As a result, all requirements and the recommendation have been restated to ensure that progress is sustained, and they will be reviewed at the next Inspection report for children’s home - monitoring visit: 2872012 3 inspection. The notice restricting accommodation will expire on 26 June 2026 and will not be reissued by Ofsted. Inspection report for children’s home - monitoring visit: 2872012 4
