Inspection report for supported accommodation provider - monitoring visit: Agape4all 2
p.2
This monitoring visit
Ofsted carried out a monitoring visit on 27 and 28 January 2026. Three compliance notices were raised under Regulations 4, 5 and 27, due to significant shortfalls in the leadership and management arrangements, the safeguarding arrangements and the support that children received. A notice restricting accommodation was also issued on 30 January 2026. This notice prevented new children from moving into the provision.
p.2
This monitoring visit
Ofsted carried out another monitoring visit on 16 and 17 March 2026. During that visit, inspectors found that insufficient progress had been made to address the concerns identified, and the restriction of accommodation notice remained in place.
p.2
This monitoring visit
On 7 and 13 April 2026, Ofsted carried out a further monitoring visit. Shortfalls in practice had not been addressed sufficiently; therefore, a further notice restricting accommodation was issued. Additional concerns were raised in relation to the safety and suitability of those responsible for the operation of the service.
p.2
This monitoring visit
This visit was carried out to monitor the provider’s progress in meeting the restriction of accommodation notice and addressing the regulatory breaches that had been identified previously.
p.2
This monitoring visit
The new manager/proposed nominated individual and a new director facilitated the monitoring visit.
p.2
This monitoring visit
The new director, new manager/proposed nominated individual, the registered service manager and the provider have failed to take effective action to improve the operation of the service. The regulatory shortfalls that were identified at the previous monitoring visits have not been addressed effectively.
p.2
This monitoring visit
Ofsted remains unsure who is actively involved and responsible for the operation of the supported accommodation undertaking. The registered service manager previously informed inspectors that the organisation had entered into a partnership agreement with 3 other individuals in January 2025. However, the provider has not formally shared the details of this arrangement with Ofsted as required or provided any assurances of the safe recruitment checks that were carried out on these individuals. The new director could not provide Ofsted with any updates on this arrangement.
p.2
This monitoring visit
The provider has failed to ensure that there are effective systems in place to ensure the safe operation of the supported accommodation service. The new director and new manager/proposed nominated could not evidence any work that they had completed in relation to incident management, notifications to Ofsted, behaviour management and the safe support of children who go missing from the home. In each case, despite the new director’s efforts to contact the registered provider/registered service manager, he and the proposed nominated individual were Inspection report for supported accommodation provider - monitoring visit: Agape4all 3 unable to provide any evidence of any work that had been completed, such as new forms to be used or training to address staff skills.
p.2
This monitoring visit
The accuracy of records remains questionable. The recruitment records of the proposed nominated individual/proposed service manager were viewed. The application form on file for this individual held incorrect details. When questioned about this, the proposed nominated individual/proposed service manager confirmed that they had not completed an application form. They confirmed that they had only submitted a CV. The new director and applicant could not explain the reason for this.
p.3
This monitoring visit
The safety of children is not promoted as the provider has again failed to address the shortfalls in recruitment practice. The updated spreadsheet to monitor staff recruitment is not up to date or accurate. Evidence that verbal verification of references is sought is not recorded on the tracker, and it is not clear if or when the status of staff’s Disclosure and Barring Service status has been checked. In addition, there were no right-to-work documents in the proposed nominated individual/proposed service manager’s recruitment file.
p.3
This monitoring visit
The provider has failed to ensure that staff receive adequate training for their role, or that identified gaps in their training are addressed appropriately and quickly. While the provider gave Ofsted assurances that staff have received training, no evidence of this could be provided. As identified in the last monitoring visit, only one member of staff had completed training in January 2026. All other training records are dated from 2025. The assertions from the new director that other staff had received additional training could not be evidenced through certification or records. Furthermore, the new director could not articulate what had been covered in the training or how staff had been assessed as competent following the training.
p.3
This monitoring visit
There was insufficient evidence of improvements to the service. The provider had told Ofsted about action plans to address shortfalls and to introduce new auditing systems. However, these were not evidenced during this monitoring visit. Inspectors were shown a standard health and safety audit from 2025. There was no evidence of any other quality audits or the monthly audit programme as referenced in the provider’s action plan. When asked about these measures, the proposed nominated individual/proposed service manager said they had not seen them. Similarly, the external quality review that was completed in January 2026 was not evidenced, and neither the proposed nominated individual/proposed service manager nor the new director could produce or describe this.
p.3
This monitoring visit
The provider has failed to demonstrate improvements to children’s support documents. The new director said that they were in discussion with a number of companies to implement new support plans, behavioural plans and risk assessments. However, inspectors saw no evidence of any company offering new support plans, what this would look like, how staff are supported to use them, or any other improvements around support planning, key working or risk assessment. Inspection report for supported accommodation provider - monitoring visit: Agape4all 4 Ofsted cannot be assured that children will receive an effective and safe service from leaders, managers and staff as record-keeping remains poor. The new director and the proposed nominated individual/proposed service manager were unable to provide inspectors with the documentation that was requested to evidence their assertions.
p.3
This monitoring visit
The provider has failed to take effective action to address maintenance issues in the premises that were first raised by Ofsted in January 2026. At the property visited, the toilet seat that had been damaged several months ago has not been repaired. Restrictive elements in the home, such as window restrictors and locks on the doors in the communal areas, remain in place.
p.4
This monitoring visit
Due to the provider’s ongoing and continued failure to meet the regulations, a further restriction of accommodation notice has been issued. Inspection report for supported accommodation provider - monitoring visit: Agape4all 5
