The service is owned and managed by a private provider. It provides 3 shared-living premises that are limited to accommodating looked-after children or care leavers.
p.1
provider
The nominated individual registered with Ofsted in January 2025. The nominated individual was not present during the visit. The director and owner facilitated this monitoring visit.
p.1
provider
There has not been a registered service manager since May 2025.
p.1
provider
At the time of this visit, the service was not providing support and accommodation for children aged 16 and 17.
p.1
provider
The provider was not given notice of the visit. One of the provider’s premises was visited, and no children were spoken with.
p.1
This monitoring visit
Ofsted carried out a monitoring visit on 11 and 12 February 2026, where significant shortfalls in the accommodation provided, the support the children receive, management of safeguarding incidents and leadership and management were identified. Four compliance notices were issued following the inspection. Ofsted carried out a further monitoring visit on 31 March 2026, where 4 requirements were found to remain. Three of the 4 compliance notices previously issued were found to have not been met. Ofsted continues to monitor this service.
p.1
This monitoring visit
This visit was carried out to review the provider’s progress with the requirements and monitor progress made since the last inspection. Since the last monitoring visit, Inspection report for supported accommodation provider - monitoring visit: Fusion Matrix Limited 2 the provider submitted a series of documents to Ofsted to demonstrate the work they had done to be compliant with regulations.
p.1
This monitoring visit
A new manager has been appointed, but they were not present on site for this visit. The director and owner has been working with the manager and nominated individual to oversee the service and ensure improvements are made. However, the manager has not applied to register with Ofsted, and recruitment files for the new manager were not available during the visit. Management oversight and systems to improve the service have not been effective, which have resulted in there being limited signs of improvement to the service. Significant concerns remain in relation to leadership and management and the safety of children.
p.2
This monitoring visit
The poor culture in the service remains, with concerns about safeguarding and the responses of staff, leaders and managers to incidents. For example, it was recorded in a child’s daily notes that police carried out a room search and removed items from a child’s room. When asked about this, the provider told inspectors that the police removed a vape pen. However, the local authority confirmed that police had removed 3 bags of cannabis. This serious event had not been recorded as an incident. There was no joint working with the local safeguarding teams or the child’s social worker. The provider had not notified Ofsted of this serious event and had failed to show professional curiosity around possible criminal exploitation.
p.2
This monitoring visit
Children’s support plans and documents were poor and had not been updated. When serious incidents had occurred, support plans, risk assessments or behavioural support plans had not been updated. This continues to put children and staff at risk, as staff do not know the most effective way to respond to children’s current and emerging needs.
p.2
This monitoring visit
The provider told inspectors that 2 of the properties were closed and returned to the landlord, but the provider has not notified Ofsted or removed them from their registration. The outside of one of these properties was in a poor state, with old and broken furniture, bedding and a staff noticeboard in the garden. The property assessment completed for one home had flooring rated as ‘Green’ meaning acceptable standard, despite the carpets being in a very poor state. Ofsted cannot be assured the provider has the management oversight and standards to ensure children’s living environments are homely.
p.2
This monitoring visit
Following this visit, 5 requirements remain and Ofsted continues to monitor this service. All the 4 compliance notices previously issued were found to have not been met. Inspection report for supported accommodation provider - monitoring visit: Fusion Matrix Limited 3
