Inspection report for residential family centre - monitoring visit: Tyre Hill House Family Assessment Centre 1 OFSTED USE ONLY
p.1
Tyre Hill House Family Assessment Centre Welland Road, Hanley Swan, Worcester WR8 0EQ
p.1
Inspected under the social care common inspection framework
p.1
Information about this residential family centre
The family centre is privately owned. It is registered to accommodate up to 8 families. The manager registered with Ofsted in January 2025.
p.1
Information about this residential family centre
There was one family living at the family centre at the time of the inspection.
p.1
This monitoring visit
The centre was judged inadequate at a monitoring visit on 18 March 2026. Due to the concerns identified during the inspection, Ofsted issued a notice restricting accommodation. Ofsted also issued 2 compliance notices under regulation 12 and regulation 8 of The Residential Family Centre Regulations 2002.
p.1
This monitoring visit
The purpose of this visit was to review the notice restricting accommodation and the progress made against the 2 compliance notices raised at the full inspection.
p.1
This monitoring visit
The family living at the centre is well supported, and managers and staff show a clear awareness of the risks and needs of the child and their parent. A social worker provided positive feedback about communication, assessment, support and safeguarding practice.
p.1
This monitoring visit
Leaders have taken proactive action to address the shortfalls identified at the last inspection. Senior leaders and managers have engaged in a period of meaningful reflection, supported by an external, independent review of safeguarding practice, safeguarding incidents and the overall functioning of the centre. As a result, the organisational structure has been strengthened with clear lines of accountability and Inspection report for residential family centre - monitoring visit: Tyre Hill House Family Assessment Centre 2 OFSTED USE ONLY

improved oversight. In addition, targeted development plans have been introduced to ensure opportunities for ongoing professional and service development and to ensure that recent improvements are sustained.
p.2
This monitoring visit
Leaders and managers have revised admission policies to ensure that risk is assessed more rigorously when considering new families. The revised policy provides clearer guidance on the information required prior to families moving into the centre. Managers have critically reflected on previous decision-making and have a stronger understanding of how these decisions compromised the safety of the families in the centre. As a result, managers are better equipped to evaluate the suitability and potential risks of new families moving into the centre.
p.2
This monitoring visit
Safeguarding policies and procedures have been reviewed and updated. They now include specific guidance about how to respond to marks and bruises on non-mobile babies. The safeguarding policy is supported by an array of quick reference flow charts to support staff to understand the actions they must take when they have safeguarding concerns or concerns about the physical or emotional health of parents.
p.2
This monitoring visit
Staff have benefited from a range of workshops and upskilling sessions. This has strengthened practice and ensured that safeguarding processes and responsibilities are understood. The introduction of a practice lead role, and practice observations, are strengthening day-to-day oversight and providing increased opportunities to monitor staff practice and address shortfalls promptly. These revised systems are in the early stages of implementation, and it is too early to see sustained or consistent impact.
p.2
This monitoring visit
Leaders and managers have strengthened recording systems and daily records held about the family. The placement plan is now a comprehensive document that outlines the family's journey and essential information about the requirements of the assessment. However, it is not yet understood how parents are helped to understand or contribute to their plan, particularly where they have specific learning needs. This has the potential to disempower parents.
p.2
This monitoring visit
Risk management plans are generally aligned with children's needs and the risks associated with parenting capacity. However, these plans do not reflect changes in risk. This limits their effectiveness in capturing the dynamic nature of risk and the ways in which risks escalate, reduce or evolve during assessment. The registered manager has already identified this, and a recommendation is made to support ongoing improvements in this area.
p.2
This monitoring visit
Leaders and managers have taken account of staffing insufficiencies. They have committed to maintaining reduced occupancy levels until sufficient staffing is secured.
p.2
This monitoring visit
Leaders have introduced some positive changes, demonstrating their commitment to improving the safeguarding practice and experiences for families. As a result of this Inspection report for residential family centre - monitoring visit: Tyre Hill House Family Assessment Centre 3 OFSTED USE ONLY
