Inspection report for supported accommodation provider - monitoring visit: Universal Healthy Minds Limited 1 Universal Healthy Minds Limited

p.1

Registered provider: Universal Healthy Minds Limited

p.1

Inspected under the social care common inspection framework

p.1

provider

This service is owned and managed by a private provider. It offers accommodation for up to 2 children in one shared group-living home. The home is not limited to accommodating looked-after children and care leavers.

p.1

provider

No children were living at the service at the time of this visit.

p.1

provider

The service manager, who is also the nominated individual and sole director, registered with Ofsted when the service was registered in March 2025.

p.1

This monitoring visit

Ofsted conducted a monitoring visit on 4 February 2026 in response to concerns raised regarding the provider’s failure to notify Ofsted of all serious events, and the suitability of children placed in the supported accommodation. Following that visit, Ofsted issued 2 compliance notices. These related to the notification of serious incidents and the referral and admission process.

p.1

This monitoring visit

A restriction of accommodation notice, in force until 30 April 2026, was also issued, to allow the provider time to make the necessary improvements to safeguard children.

p.1

This monitoring visit

A further monitoring visit was undertaken on 31 March 2026 to monitor the provider’s progress towards meeting the compliance notices and the restriction notice. Sufficient evidence was found that the provider had met the requirements of the compliance notices. However, concerns remained regarding the maintenance and safety of the accommodation. Inspection report for supported accommodation provider - monitoring visit: Universal Healthy Minds Limited 2 The purpose of this monitoring visit was to consider the progress made against the restriction notice. The inspector visited the premises and spoke with a member of staff and a relative of the nominated individual. They also spoke with another member of staff by phone. The members of staff confirmed that no children had been admitted since the last child had moved out of their accommodation, before the monitoring visit on 31 March 2026.

p.1

This monitoring visit

The inspector was unable to contact the nominated individual, who is also the registered service manager, during the visit. There is no arrangement in place to ensure that the service is overseen by a competent person in the absence of the registered service manager. Staff spoken with had limited knowledge of the service. Staff were unable to provide relevant evidence to demonstrate that it would be safe to accommodate children.

p.2

This monitoring visit

Leaders and managers have failed to demonstrate effective oversight of the service. Health and safety concerns raised previously about the premises have not been rectified. For example, the fence and gate at the rear of the property are damaged, making the garden hazardous and accessible to the general public. The premises is not well maintained. It is not a welcoming and homely environment for a child. For example, one bedroom has a stained carpet and the walls are damaged, and another has a damaged door.

p.2

This monitoring visit

Following the restriction of accommodation notice served in February 2026, the provider invited staff to secure employment elsewhere while the restriction was in place and there were no children. Leaders and managers have failed to demonstrate there are sufficient suitably trained staff to operate the service safely, should a child be accommodated.

p.2

This monitoring visit

Following this visit, a case review was held, and it was agreed that the provider had not demonstrated they had met the requirements of the restriction notice. Therefore, a further restriction notice has been served to ensure that no children are accommodated.

p.2

This monitoring visit

One requirement made at the last visit is outside the scope of this visit and will be reviewed at the next inspection. Inspection report for supported accommodation provider - monitoring visit: Universal Healthy Minds Limited 3
