Inspection report for children's home - monitoring visit: SC396463 1 SC396463 Registered provider: Cambian Childcare Limited

Inspected under the social care common inspection framework
p.1
Information about this children’s home
This home is privately owned and registered to provide care for up to 2 children with social and emotional difficulties.
p.1
Information about this children’s home
The registered manager was not present during the visit.
p.1
Information about this children’s home
One child was living in the home at the time of the visit. The child talked with the inspectors.
p.1
This monitoring visit
On 10 and 11 March 2026, a full inspection was conducted, which identified serious and widespread concerns regarding safeguarding practices and the leadership and management of the home. As a result, a compliance notice was issued under Regulation 12 of The Children’s Homes (England) Regulations 2015, directing the provider to implement immediate actions to improve the safety and the experience of the child living in the home.
p.1
This monitoring visit
This monitoring visit was carried out to assess whether sufficient action had been taken to meet the compliance notice in relation to Regulation 12 to improve the safety and experiences of the child living in the home. The registered provider has complied with the restriction of accommodation notice, and no children have moved into the home. The registered person has not taken sufficient actions to meet the steps set out in the compliance notice.
p.1
This monitoring visit
Inspectors found that the compliance notice had not been met. Despite increased senior leadership oversight, enhanced management involvement and additional staff training, staff continue to demonstrate poor interactions and an inconsistent approach. This continues to cause the child unnecessary distress. As a result, staff responses are still contributing to significant incidents in the home, placing both the child and staff in unsafe situations and at risk of harm. Inspection report for children's home - monitoring visit: SC396463 2 Behaviour management practice continues to be inconsistent. Situations that could reasonably have been anticipated or managed through preparation and predictable routines still escalate unnecessarily, including incidents resulting in damage to property. For example, staff did not ensure the child had what they needed for their morning routine. This led to an avoidable incident that caused the child distress and
p.1
This monitoring visit
undermined their sense of emotional stability. These inconsistencies create instability for the child, increase levels of distress and hinder the development of safe, trusting relationships with staff.
p.2
This monitoring visit
The provider has begun to address the previously identified shortfall regarding staff understanding of roles and responsibilities in relation to reporting and recording. Staff have received training on recording, reporting and sharing concerns. Despite this, there remain gaps in the quality, accuracy and reliability of incident records. Important information is either missing or not clearly documented. For example, when the child has been physically restrained by leaders, the records do not consistently reflect this. On occasions when the child raised concerns about leaders’ actions during physical interventions, the same leader spoke to the child about the incident. This practice does not provide independence or enable the child to speak openly, significantly reducing the effectiveness of safeguarding arrangements.
p.2
This monitoring visit
Progress in investigating incidents where the child has made allegations or raised concerns about staff practice remains insufficient. Incidents that should have been reviewed have not been subject to robust managerial scrutiny. Although some historical concerns have been shared with the local authority designated officer, there is limited evidence of a thorough investigation, evaluative oversight or meaningful enquiry into concerns raised by the child. Without reliable investigation and review, safeguarding concerns are not fully understood or addressed. This exposes the child to continued risk and restricts their ability to have their voice heard and taken seriously.
p.2
This monitoring visit
Leaders have taken some steps to improve staff understanding of the child’s needs by conducting therapeutically informed training. Some of the information from the training has been added to the child’s plans. However, key information is still not consistently reflected across risk assessments and care-planning documents. As a result, staff do not have access to clear, up-to-date guidance to support consistent and safe practice.
p.2
This monitoring visit
There is limited evidence that learning from training and previous incidents has informed staff practice. Incidents occurring since the last visit demonstrate a repetition of previous shortfalls, including inconsistent behaviour management and weak oversight. This indicates that leaders and managers have not yet ensured that improvements are sustained or effectively implemented.
p.2
This monitoring visit
The limited and ineffective action taken to date does not provide Ofsted with assurance that leaders, managers or staff have the capacity, competence or skills necessary to ensure the child’s safety. Following the inspection, the child moved from the home. Further compliance notices have been issued requiring the provider Inspection report for children's home - monitoring visit: SC396463 3 to take actions to improve the quality of care provided to children before any further children are admitted to the home. Requirements issued at the previous full inspection have not been reviewed as part of this visit and, as a result, remain in place.
