Inspection report for supported accommodation provider - monitoring visit: Agape4all 1 Agape4all
p.1
Inspected under the social care common inspection framework
p.1
provider
The service is owned and managed by a private provider. It provides 3 shared-living premises that are limited to accommodating looked-after children or care leavers.
p.1
provider
The service manager, who is also the nominated individual and director was in position before the service registered with Ofsted in September 2024.
p.1
provider
At the time of this visit, no children were living with the service. The last child moved out on 03 March 2026.
p.1
provider
The provider was not given notice of the visit, and one of the provider’s premises was visited.
p.1
This monitoring visit
Ofsted carried out a monitoring visit on 27 and 28 January 2026. Due to significant shortfalls in the leadership and management arrangements, affecting the safeguarding arrangements and the support that children received, 3 compliance notices under regulation 4, 5 and 27 were raised. A notice restricting accommodation was also issued. This notice prevented new children from moving into the provision.
p.1
This monitoring visit
This monitoring visit was carried out to monitor the provider’s progress in meeting the steps identified in the compliance notices and to see what progress had been made in addressing the concerns leading to the restriction of accommodation notice.
p.1
This monitoring visit
No children have been admitted since the notice restricting accommodation was issued. Inspection report for supported accommodation provider - monitoring visit: Agape4all 2 The provider has operated from a premises that they have failed to Ofsted about. This is a breach of their conditions of registration. Furthermore, the service manager failed to inform Ofsted of this premises address or share this information about the child’s placement with the inspectors during this and the previous monitoring visit in January 2026.
p.1
This monitoring visit
The quality of the accommodation remains poor. The premises visited was cold and unwelcoming.
p.2
This monitoring visit
Governance and quality assurance systems remain weak. The service manager said that they had an action plan to demonstrate how they had met the steps in the compliance notices raised under regulation 4, 5 and 27. However, they failed to produce this. Instead, a copy of an operational review meeting was shared. This contained action points that had not been completed. The manager was unclear about what an audit is.
p.2
This monitoring visit
Ofsted cannot be assured that the service manager has a suitable action plan in place to support ongoing developments in the service. While the service manager said that she had submitted the required quality of support review that is overdue. The last regulation 32 report that Ofsted received was signed and dated on 29 May 2025. This was received by Ofsted on 1 July 2025.
p.2
This monitoring visit
Management oversight remains inadequate. The service manager could not articulate the support that the one child had received. The service manager frequently relied on other staff to provide information on one child’s support and incident management. Furthermore, the manager has failed to ensure that staff understand the importance of and have the skills to record all serious incidents involving children. This oversight prevented staff and managers from providing clear and accurate information on what action had been taken or what future interventions the child needed to remain safe.
p.2
This monitoring visit
Support for children remains poor. Leaders and managers have failed to ensure that children received the support that they required to meet their changing needs. One child’s support plan, risk assessment and behaviour plan have not been updated following significant incidents. This prevents staff from understanding how they must support and respond to the needs of the child. Leaders and managers have failed to identify or put right these shortfalls.
p.2
This monitoring visit
The experiences of children are unclear. Inspectors found it difficult to obtain the children’s records or view documents pertaining to the operation of the service. The service manager told the inspectors that she kept the children’s records locked at her home. The documents that the manager agreed to send to the inspectors, such as daily notes and action plans were not received.
p.2
This monitoring visit
The service manager could not provide an incident report identifying why a child had breached their bail conditions. The record that was provided for this incident, was for a different incident involving the child’s arrest later that day. The lack of complete Inspection report for supported accommodation provider - monitoring visit: Agape4all 3 and accurate records for children means that Ofsted cannot be satisfied that children are effectively safeguarded when they are in or away from their home.
p.2
This monitoring visit
The service manager has failed to demonstrate how they and the staff have supported and educated one child to make safer and better choices. This child has not been afforded any time or space to talk to the staff about their behaviour and actions since the previous monitoring visit in January 2026.
p.3
This monitoring visit
The service manager has failed to demonstrate that the staff have the skills and experience they need to support children effectively and safely. The training matrix shared with the inspectors was not up to date. Furthermore, when additional training has been identified for staff, the service manager has failed to provide these staff with relevant training opportunities.
p.3
This monitoring visit
The provider has developed a tool to assess any gaps in the staff’s employment or academic history. However, this tool has not been expanded so that gaps in staff’s references, verification of references, criminal record checks, VISA or right to work can be identified. Consequently, the provider cannot be confident that all the current staff are able to safely meet the needs of children within the service.
p.3
This monitoring visit
The provider has notified Ofsted of serious incidents as required by legislation, and shared information with some other agencies. However, the overall management of incidents was poor, and there was a lack of action and accountability from leaders and managers.
