p.1

Inspection date: 18 March 2026

Date of previous inspection: 9 December 2025

p.1

This monitoring visit

This visit was carried out in response to information received by the regulator that raised concerns about the safeguarding culture within the centre. These concerns related to care planning arrangements for families moving to the centre, failure to follow safeguarding processes and the effectiveness of leaders and managers to oversee safeguarding practice.

p.1

This monitoring visit

Leaders and managers’ care planning processes and decision-making are poor. They do not prepare or assess information effectively to ensure they can meet the needs of the families living at the centre. Managers do not use their professional curiosity effectively to ensure that risks are understood and that staff have the skills to respond to these risks. In addition, one parent was not adequately prepared for moving into the centre, significantly reducing their understanding and ability to make an informed decision about undertaking an assessment. This created a disadvantage from the outset. These shortfalls reflect significant weaknesses in strategic oversight and a failure to prioritise the safety and welfare of families. Inspection report for residential family centre – monitoring visit: Tyre Hill House Family Assessment Centre 2 Managers and staff do not implement appropriate safeguarding measures. They

p.1

This monitoring visit

have not provided sufficient support, supervision and monitoring of a vulnerable parent, leaving them unsupervised and unsupported when they faced complex challenges. As a result, a very vulnerable parent went missing, and managers and staff were unaware for over 12 hours. In addition, they did not inform the relevant professionals in the local authority at the earliest opportunity.

p.2

This monitoring visit

Inaccurate guidance and a lack of clear direction contributed to a missed opportunity to ensure a child’s safety. Senior leaders did not follow the centre’s safeguarding processes in relation to an unexplained mark on a child. As a result, opportunities to fully explore whether the mark indicated harm were missed. The investigation into this incident has not ensured enough scrutiny or been undertaken by someone who is impartial and independent. This has compromised opportunities to identify learning, address poor practice and take appropriate action to safeguard children.

p.2

This monitoring visit

Significant weaknesses in leadership and management have compromised the safety and welfare of families. Leaders and managers do not explore risk adequately and ensure that they work in partnership with safeguarding professionals, including the local authority designated officer. This undermines confidence in leaders and managers’ ability to follow safeguarding processes and limits prompt external oversight and independent assessment of potential risk.

p.2

This monitoring visit

Staff do not understand their safeguarding roles and responsibilities. Those staff responsible for waking night shifts have been sleeping during the night rather than fulfilling the duties associated with their role. This poor practice places children at risk.

p.2

This monitoring visit

When serious incidents occur, records do not provide the level of detail required to ensure effective safeguarding oversight. Records fail to provide an accurate account of the incident, actions taken and rationale for decision-making. As a result, leaders and managers cannot reliably evidence that risks have been assessed or managed appropriately.

p.2

This monitoring visit

Leaders and managers have not ensured that staff have the sufficient skills, experience and confidence to meet the needs of families and keep them safe. While the centre is registered to support up to 8 families, staff have reported challenges when occupancy has increased to 3 families. As a result, agency staff and managers are required to cover gaps in the rota. This hinders the capacity of managers to oversee practice at the centre and does not provide reassurances that there are sufficient staff to safely care for children and their families.

p.2

This monitoring visit

As result of the concerns identified at this inspection, senior leaders implemented an action plan and took action to ensure the immediate safety of families living at the centre. Inspection report for residential family centre – monitoring visit: Tyre Hill House Family Assessment Centre 3
