Inspection report for supported accommodation provider - monitoring visit: Avantis Living 1 FOR PUBLIC RELEASE

p.1

Registered provider: Avantis living Limited

p.1

Inspected under the social care common inspection framework

p.1

provider

A private provider owns this supported accommodation service. It offers accommodation in 16 properties registered to deliver supported accommodation in a shared or group living situation in premises used to accommodate looked-after children or care leavers. The registration also includes premises that are not limited to accommodating looked-after children and care leavers.

p.1

provider

At the time of this inspection, 8 looked-after children or care leavers were using this service.

p.1

provider

The organisation and the service manager registered with Ofsted in July 2025. The same person holds the nominated individual and registered service manager roles.

p.1

This monitoring visit

Ofsted carried out a full inspection across this service from 12 to 16 January 2026. The inspectors found serious and widespread weaknesses, leading to significant concerns about the leadership and management of the service.

p.1

This monitoring visit

Following that inspection, Ofsted issued 4 compliance notices. These related to the leadership and management of the service, the standard of children’s accommodation, the notification of serious incidents and the support to children.

p.1

This monitoring visit

The purpose of this monitoring visit was to consider the progress made against the compliance notices. This included discussions with the registered service manager, re-visiting one of the premises and reviewing the children’s case records and the records relating to the leadership and management of the service. Inspection report for supported accommodation provider - monitoring visit: Avantis Living 2 FOR PUBLIC RELEASE

p.1

This monitoring visit

Since the inspection, 2 children have been discharged from the service, and one child has been admitted.

p.2

This monitoring visit

Inspectors found that the registered service manager, who is also the nominated individual, has failed to maintain suitable health and safety standards. In the premises visited, they have neglected to ensure that the fire door leading to the cellar closes shut. This presents a fire safety risk. Additionally, there is nothing to prevent children from entering the cellar, despite this being unsafe and hazardous. The fire door closure has not been fixed to stop it from clashing with the ceiling lighting when opening the fire door. This presents a serious risk to children. Due to the severity of the concern, Ofsted has taken further enforcement action to prevent the use of the property.

p.2

This monitoring visit

The provider has repeatedly failed to provide children with a consistently welcoming, homely, clean and well-maintained environment. The damage around the property previously identified has still not been repaired. The dining rooms located on the ground and first floor were sparsely and poorly furnished. They were not homely and nurturing environments.

p.2

This monitoring visit

The dining furniture was of poor quality. This included a badly damaged dining table on the ground floor that had not been replaced. There was a limited number of dining chairs available. The garden at the rear of the property was poorly maintained. These failures continue to show a lack of value for children. As a result of these significant shortfalls, the compliance notice has been reissued.

p.2

This monitoring visit

The new referral, admission and compatibility process is a work in progress. It fails to include the rationale for how placement decisions are made. The manager has introduced a new electronic recording system to improve the management oversight of the service. It is still in its early stages, and it does not include a detailed oversight of all children’s progress and safeguards. No system is in place to ensure that all children placed in any new premises are notified to Ofsted. The compliance notice is not met, and it has been reissued.

p.2

This monitoring visit

The new system to monitor and escalate serious incidents to Ofsted have led to an increase in the number of notifications submitted to Ofsted. This reflects the provider’s improved understanding of what is required. Children’s plans have been improved to ensure they are accurate, up to date and offer clearer guidance to the staff. An escalation policy has been devised. These compliance notices are met.

p.2

Requirements made at the last inspection are outside the scope of this visit. These will be reviewed at the next inspection. Inspection report for supported accommodation provider - monitoring visit: Avantis Living 3 FOR PUBLIC RELEASE
