Inspection report for children’s home - monitoring visit: 2747219 1 Registered provider: Madiba Yorkshire Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

The home is owned and managed by a private provider. It provides care for up to three children who may experience social and emotional difficulties, and who may have learning disabilities.

p.1

Information about this children’s home

There has been no registered manager at the home since 16 October 2025. There is an interim manager in post who is still to make an application with Ofsted.

p.1

Information about this children’s home

There was one child living in the home. At the time of this monitoring visit, the child was being accommodated at a location away from the home and care was provided by agency staff recruited by the child’s placing authority. The child was, therefore, not seen or spoken to by the inspectors.

p.1

This monitoring visit

This visit was undertaken to address specific concerns or allegations received by Ofsted. This relates to staff’s use of physical restraint, the manager’s oversight of the recording of these incidents, and allegation management.

p.1

This monitoring visit

Records show that staff need to physically hold or guide the child when they use risky and unsafe behaviours. Staff’s intervention has been necessary to keep the child and others safe.

p.1

This monitoring visit

Staff’s recording of the incidents when physical restraint has been used is not recorded in line with the regulations. This is because records do not always provide a clear description of how the child has been held, or the sequence of events when multiple holds have been used. This is especially when incidents have been prolonged. Inspection report for children’s home - monitoring visit: 2747219 2 Furthermore, staff and the child are not always provided with the opportunity to reflect on what has happened. This includes: not having the chance to discuss what actions staff have taken to try and keep the child calm; what has caused the child to become upset; why staff have felt the need to either hold or guide the child; what holds have been used; and how the incident has affected the child and staff.

p.1

This monitoring visit

The failure of managers to speak to the child and staff involved means there is no opportunity to reflect on how practice can be changed, so that staff can keep the child calmer and to reduce the need of physical intervention.

p.2

This monitoring visit

An allegation made by the child was shared with safeguarding professionals and the police. However, there are shortfalls in how the allegation has been managed. Records are poor, making it difficult to determine what action has been taken and why, as well as what discussions have taken place.
