Information about this inspection
the care of children the safety of children the effectiveness of leaders and managers. The inspector has looked closely at the experiences and progress of children, using the social care common inspection framework. This assurance inspection was carried out under the Care Standards Act 2000 to assess the effectiveness of the service, how it meets the core functions of the service as set out in legislation, and to consider how well it complies with The Children's Homes (England) Regulations 2015 and the 'Guide to the Children's Homes Regulations, including the quality standards'. p.1
Findings from the inspection
This assurance inspection was carried out to review the provider's response to serious incidents that have been notified to Ofsted. We identified serious and widespread failings in the care and protection provided to the previous child living in the home.
p.2
Findings from the inspection
Leaders and managers learned about the child's needs prior to their move into the home. However, they did not ensure that staff had the skills, knowledge and experience required to meet the child's evolving needs. Significant gaps in staff competence were identified in relation to the supervision of the child and their understanding of digital safety, missing-from-home procedures and risk management. These shortfalls contributed to the child moving on from the home in an unplanned way.
p.2
Findings from the inspection
Inadequate staff supervision allowed the child to access the home's mobile. Leaders and managers were unable to provide a clear explanation as to how the child unlocked the mobile phone. Staff did not manage known risks and supervise the child in line with their vulnerabilities and care plan.
p.2
Findings from the inspection
On another occasion, staff did not follow the agreed missing-from-home protocol, resulting in a delay in staff reporting the child as missing to the police. The child subsequently came to significant harm while they were missing from home.
p.2
Findings from the inspection
Hazardous items have not been stored safely and securely by staff. The child was able to gain possession of a sharp kitchen knife. Given the child's vulnerabilities and incidents of self-harm the failure to secure this item promptly created an unnecessary and foreseeable risk.
p.2
Findings from the inspection
Not all staff had completed the required training to help them understand and meet the child's specific needs. This meant that they were not fully prepared or equipped to carry out their roles safely or effectively.
p.2
Findings from the inspection
Staff have not kept the personal information of others confidential. Ineffective systems and weakness in staff knowledge and practice led to sensitive information being shared and not safeguarded.
p.2
Findings from the inspection
The recommendations raised at the last inspection in relation to staff having the necessary qualification to inform their practice and children’s unsafe access to devices have not been met. As a result, requirements have now been raised, one of which is subject to a compliance notice. Assurance inspection report for children's home: 2841616 3
