p.1
This monitoring visit
An assurance visit took place on 8 December 2025, where there were serious and widespread concerns relating to children’s safety and the leadership and management of the home. As a result, compliance notices were issued under Regulation 12, ‘The protection of children standard’, and Regulation 13, ‘The leadership and management standard’. On 20 January 2026, a monitoring visit took place.
p.1
This monitoring visit
During the monitoring visit it was identified that the organisation had not met the steps in the compliance notices. The decline in the level of care being provided to children was very concerning, leading to the registration of the home being suspended. The purpose of this visit was to monitor compliance with the suspension notice and to explore how the organisation is meeting other regulatory requirements. The organisation is adhering to the suspension of the registration. Inspection report for children’s home - monitoring visit: 2770974 2 Following the last inspection and the imposition of the suspension, two children moved out of the home. No children have moved in. The home has remained closed since the two children moved out.
p.1
This monitoring visit
Leaders and managers are aware that further work is needed to ensure that children can live safely at the home. Senior leaders were honest about what has been completed so far.
p.2
This monitoring visit
The organisation has not been able to demonstrate that sufficient progress has been made. There is no management team or staff team. The organisation has begun a recruitment campaign to source a manager; however, no offer of employment has been made. A senior manager advised that there is no plan to open the home until a manager is in place.
p.2
This monitoring visit
All the members of the staff team have been redeployed to other homes on the same site. There has been no learning exercise with any of them, either individually or as a group, to consider the events that led up to the suspension. Staff have not been provided with time to reflect on their experiences. This means staff have not had an opportunity to discuss their concerns and improve their practice.
p.2
This monitoring visit
Staff from the home have not been provided with supervision sessions to help them understand what is required of them in their roles in other services. This is a missed opportunity by managers to develop and improve staff practices.
p.2
This monitoring visit
Senior managers have not reviewed the serious incidents that led up to the suspension, despite these being referred to in several notifications received by Ofsted after the inspection. This means that senior managers cannot be sure that staff who are working in other homes have always responded to children in safe ways.
p.2
This monitoring visit
At time of the last inspection, the manager was under a performance improvement plan. Since the last inspection, the manager has been continuing to have management responsibilities, including providing on-call advice and carrying out investigations. However, the performance plan has not been reviewed at all. This means that despite clear issues with the manager’s capability, particularly in relation to keeping children safe, there has been no senior management oversight to ensure learning and accountability. This has led to the manager contributing to a significant safeguarding incident in another home involving two children. Senior managers have not ensured that poor management practice is effectively responded to.
p.2
This monitoring visit
Some staff are currently under investigation, and they are still working in other homes on the site. At the time of the monitoring visit, a senior manager advised that one investigation had been completed. However, the report demonstrated gaps in the investigation process that have not been followed up. This means that the investigation process is not rigorous and investigating officers are not skilled at ensuring that full details are collected to fully learn from incidents and to ensure fair outcomes for staff. The details of this investigation and the staff member involved has not been notified to Ofsted. Inspection report for children’s home - monitoring visit: 2770974 3 Some staff have received training; however, this is only evidenced for staff who are working at one of the homes. This means that, despite working in other homes, it isn’t clear if the members of the staff team are trained to provide effective care.
p.2
This monitoring visit
Senior managers have not fully understood the serious failings that led to the suspension. Staff and the manager have continued working in other homes without reflection and learning being completed. This has contributed to continued further harm being caused to children in other homes. Inspection report for children’s home - monitoring visit: 2770974 4
