Inspection report for children’s home – monitoring visit: 2777336 1 Registered provider: AVB Enterprises Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

This home is owned by a private provider and is registered to care for up to three children who may require support with social and emotional needs.

p.1

Information about this children’s home

The manager registered with Ofsted in March 2026.

p.1

Information about this children’s home

There was one child living at the home at the time of the visit, and their views were sought.

p.1

This monitoring visit

This home was found to have serious and widespread concerns at the assurance inspection on 20 October 2025. Following the inspection, two statutory requirements were set, and two compliance notices were issued in relation to regulation 12, the protection of children standard and regulation 13, the leadership and management standard. Ofsted issued a restriction of accommodation notice to ensure that no further children were admitted, given the identified concerns.

p.1

This monitoring visit

A monitoring visit was carried out on 3 December 2025 which identified that the compliance notices and statutory requirements set following the assurance inspection, had not been met. Four statutory requirements were re-set, and the restriction of accommodation notice was re-issued.

p.1

This monitoring visit

A further monitoring visit was carried out on 28 January 2026 which identified that the statutory requirements set following this monitoring visit were not met and were re-set. The restriction of accommodation notice remained. Inspection report for children’s home – monitoring visit: 2777336 2 This visit was carried out to evaluate whether the requirements set following the previous monitoring visit had been met and to review compliance with the restriction of accommodation notice.

p.1

This monitoring visit

Since the last inspection, one child has moved out in an unplanned way following an escalation in serious incidents. Therefore, one child remains.

p.2

This monitoring visit

At the last inspection, a home manager had been appointed. The manager has since successfully registered with Ofsted in March 2026.

p.2

This monitoring visit

Assessments of risk are regularly reviewed, updated and contain the most recent information, with a clear plan for staff to follow. However, staff fail to follow the agreed safeguarding procedures consistently during incidents. For example, staff do not routinely seek medical advice if a child is suspected of using substances. Staff also failed to follow agreed protocols when a child was at risk of self-injurious behaviours, resulting in a further injury being sustained.

p.2

This monitoring visit

The manager is prioritising learning and reflection with the staff team and has increased the frequency of team meetings. However, this remains a new development and is not yet fully embedded. Alongside this, staff supervision is not always completed within required timescales, as set out in the statement of purpose. This is a missed opportunity to strengthen staff reflection and promote consistent practice.

p.2

This monitoring visit

Since the last inspection, the use of physical intervention has reduced significantly. Staff recording of incidents has been strengthened. However, the post-incident review of incidents remains a shortfall. Children and staff are not consistently spoken to following incidents. Furthermore, in one incident, there was an irregularity in staff recordings, this had not been identified or addressed by leaders and managers. Therefore, this limits the managers ability to assure themselves and the wider professional network, that the intervention was appropriate or proportionate.

p.2

This monitoring visit

Leaders and managers have failed to ensure that two serious events were notified to Ofsted, in line with regulatory requirements.

p.2

This monitoring visit

The manager has strengthened their oversight following incidents. A barrier in the online recording system has been identified, which has impacted the recording of management oversight. This has been addressed and a different recording system has been identified and is in the process of being implemented.

p.2

This monitoring visit

There has been some progress in meeting the statutory requirements that were set at the previous inspection. However, these requirements are re-set. The restriction of accommodation notice has been lifted. Inspection report for children’s home – monitoring visit: 2777336 3
