At these inspections, the inspector evaluated:

p.1

Information about this inspection

 the care of children  the safety of children  the effectiveness of leaders and managers. Inspectors have looked closely at the experiences and progress of children, using the social care common inspection framework. This assurance inspection was carried out under the Care Standards Act 2000 to assess the effectiveness of the service, how it meets the core functions of the service as set out in legislation, and to consider how well it complies with The Children's Homes (England) Regulations 2015 and the 'Guide to the Children's Homes Regulations, including the quality standards'. Assurance inspection report for children's home: 2747747 2

p.1

Findings from the inspection

This assurance inspection was carried out following concerns Ofsted had regarding care of the child. We identified the following serious and widespread concerns in relation to the care or protection of children at this assurance inspection:

p.2

Findings from the inspection

 Leaders and managers have failed to effectively report and manage a child's allegation in line with the home's safeguarding policy

p.2

Findings from the inspection

 Leaders and managers have not had effective oversight of physical restraint

p.2

Findings from the inspection

As a result of this assurance inspection, Ofsted issued a compliance notice in respect of regulation 12, the protection of children standard. Ofsted will monitor the provider's compliance with the notice to determine if any further action is necessary.

p.2

Findings from the inspection

Leaders and managers failed to share an allegation of harm made by the child against two members of staff. The information was not shared with the child's social worker or the local authority designated officer (LADO). The lack of timely reporting meant crucial safeguarding actions were delayed.

p.2

Findings from the inspection

The responsible individual and registered manager regarded the child's allegation as a complaint and commenced an internal investigation without any consultation with the child's placing authority and LADO. This undermines the home's own safeguarding policy and does not promote the child's safety in the home. Delays in this process meant the child was not given prompt assurance that their concerns had been addressed.

p.2

Findings from the inspection

Additionally, leaders and managers failed to notify Ofsted about the allegation. This follows a requirement set at the two previous full inspections to report allegations to Ofsted. This reduces the regulator's ability to scrutinise the child's safeguarding arrangements.

p.2

Findings from the inspection

The use of physical restraint has not been risk assessed in line with the child's individual needs and diagnoses. As a result, staff have an inconsistent understanding of the circumstances when this intervention should be used. This places the child at potential risk of harm.

p.2

Findings from the inspection

When physical restraint is used, leaders and managers do not speak to all the staff involved after the incident. This limits the manager's oversight and opportunities to address any shortfalls in practice.

p.2

Findings from the inspection

Two requirements set at the last full inspection have not been met, and another requirement and recommendation were outside of the scope of this inspection. As a result, these will be restated, along with two additional requirements to address the shortfalls identified in this inspection. Assurance inspection report for children's home: 2747747 3
