Inspection report for children's home - monitoring visit: 2777336 1
p.1
AVB Enterprises Limited
Inspected under the social care common inspection framework
p.1
Information about this children's home
The home is owned by a private provider and is registered to provide care for up to three children who may require support with social and emotional needs.
p.1
Information about this children's home
The registered managers post has been vacant since September 2024. There is an interim manager in post who has not yet registered with Ofsted.
p.1
Information about this children's home
There were two children living at the home at the time of the visit, and their views were sought.
p.1
This monitoring visit
This home was found to have serious and widespread concerns at the assurance inspection on 20 October 2025. Following the inspection, two statutory requirements were set, and two compliance notices were issued in relation to regulation 12, the protection of children standard and regulation 13, the leadership and management standard. Ofsted issued a restriction of accommodation notice to ensure that no further children were admitted, given the identified concerns.
p.1
This monitoring visit
A monitoring visit was carried out on 3 December 2025 which identified that the compliance notices and statutory requirements set following the assurance inspection, had not been met. Four statutory requirements were re-set, and the restriction of accommodation notice was re-issued.
p.1
This monitoring visit
This visit was carried out to evaluate whether the requirements set following the monitoring visit had been met. Inspection report for children's home - monitoring visit: 2777336 2 There has been no registered manager in post since September 2024. Three managers appointed have failed to register with Ofsted. At the time of the visit, another manager has been appointed and has submitted their application to register with Ofsted. The responsible individual has failed to effectively supervise the management team during these times of challenge. There was no evidence of the responsible individual’s oversight during this inspection period.
p.1
This monitoring visit
Although staff recording of safeguarding incidents has improved and now contains more detail, leaders and managers do not consistently provide effective oversight. Staff failures to follow agreed safeguarding procedures are not routinely identified or challenged. Consequently, leaders and managers do not take timely action to address shortfalls in staff practice.
p.2
This monitoring visit
The new manager is taking steps to promote learning and reflection with the staff team. However, these actions are at an early stage and have not yet resulted in sustained improvements in practice. While a recording shortfall has been discussed in a team meeting, and a learning review of an incident has been completed, the impact of this is limited as learning from incidents has not yet been embedded into staff practice.
p.2
This monitoring visit
When an incident indicates that a child is at risk of harm, the manager appropriately shares the information with safeguarding partners. However, actions agreed through multi-agency discussions are not progressed in a timely way. Leaders and managers fail to challenge delays by placing authorities and do not put appropriate safeguarding measures in place whilst awaiting formal responses.
p.2
This monitoring visit
Assessments of risk are not regularly reviewed and updated. They do not contain all accurate or up to date information. As a result, staff do not have a clear understanding of children’s risks and therefore respond inconsistently to incidents. For example, risks associated with a child’s escalating substance misuse are not appropriately identified or managed. Consequently, risks linked to criminal exploitation are increasing and children are not adequately protected.
p.2
This monitoring visit
Since the last inspection, there has been an increase in the use of physical intervention. Although staff recording of these incidents has improved and now includes greater detail, leaders and managers fail to ensure effective post-incident review. Children and staff are not consistently spoken to following incidents which limits opportunities for reflection and learning. The manager does not provide formal oversight of physical intervention records. As a result, safeguarding partners cannot be assured that interventions are appropriate, necessary or proportionate.
p.2
This monitoring visit
One child has a significant health condition and has recently undergone two significant surgical procedures. Leaders and managers have failed to ensure that the use of physical intervention for this child has been considered within a multi-agency forum. The manager has not sought advice from medical professionals or physical Inspection report for children's home - monitoring visit: 2777336 3 intervention trainers to assess whether any holds used are safe and appropriate considering the child’s complex medical needs.
p.2
This monitoring visit
Leaders and managers have not ensured that all serious events have been notified to Ofsted in line with regulatory requirements.
p.3
This monitoring visit
The statutory requirements are considered not met and are re-stated. Therefore, the restriction of accommodation notice remains in place.
