p.1

Information about this inspection

 the care of children  the safety of children  the effectiveness of leaders and managers. Inspectors have looked closely at the experiences and progress of children, using the social care common inspection framework. This assurance inspection was carried out under the Care Standards Act 2000 to assess the effectiveness of the service, how it meets the core functions of the service as set out in legislation, and to consider how well it complies with The Children’s Homes (England) Regulations 2015 and the ‘Guide to the Children’s Homes Regulations, including the quality standards’. Assurance inspection report for children’s home: 2845569 2

p.1

Findings from the inspection

We identified serious and widespread concerns in relation to the care and protection of children at this assurance inspection.

p.2

Findings from the inspection

This assurance inspection was carried out on 11 December 2025 following a whistleblowing concern reported to Ofsted regarding the child’s safety. Inspectors identified serious failings in safeguarding, welfare and leadership within the home.

p.2

Findings from the inspection

The child has autism and sensory needs and was found to have been chewing a lead connecting the shower box to the shower head. Although staff had raised this concern with the manager, no immediate action was taken to stop the child from using the shower or to implement urgent measures to reduce the unknown level of risk.

p.2

Findings from the inspection

The child has a known nut allergy, and the provider had implemented a strict procedure for monitoring all food items entering the home to minimise risk. This included documentation stating that, ‘To protect all children within the home, including a child with a severe airborne nut allergy, all visitors, including staff, are fully informed of the nut ‑ free policy, risk management measures, and documentation requirements.’ However, products found in the home at the time of the inspection were labelled as ‘may contain nuts’, which could have posed a significant health risk. The manager accepted that further work was required to review and strengthen this area of practice.

p.2

Findings from the inspection

Additional concerns include unsafe bedroom arrangements as the child’s bedroom door lock lacks a thumb turn mechanism and is key locked. There is evidence of poor hygiene, with a strong smell of urine, and the child’s toilet was filled with excrement at the time of the inspection. Furthermore, there is a damaged ligature blanket on the child’s bed, which has not been replaced.

p.2

Findings from the inspection

Staff have not received training to meet the child’s communication needs, despite the home being registered to care for children with learning disabilities and social and emotional difficulties. Staff were unable to explain how the child’s needs are understood or supported, increasing the risk of misinterpretation and distress.

p.2

Findings from the inspection

There are also inconsistencies within the staff team and insufficient staffing levels to safely care for 2 children, as required by the home’s registration. Conflict between team members is further undermining stability and consistency for the child.

p.2

Findings from the inspection

These failings place the child at risk of serious and avoidable harm, compromise their right to a safe and dignified environment and reflect unsafe organisational practice. Assurance inspection report for children’s home: 2845569 3
