p.1

This monitoring visit

This visit was undertaken to review the provider’s compliance with two compliance notices, reissued following a monitoring visit on 29 October 2025. A restriction on new children moving into the service was issued following the 11 September 2025 monitoring visit.

p.1

This monitoring visit

The registered service manager continues to be absent from the service. Inspectors were unable to establish the exact length of time that the registered service Inspection report for supported accommodation provider - monitoring visit: Sunset Rehabilitation Healthcare Centre and Trading Associates Ltd manager has not provided management oversight. However, there was no evidence of their involvement in the running of the service since July 2025.

p.1

This monitoring visit

The nominated individual and another manager continue to oversee the service. They demonstrate limited knowledge of the supported accommodation regulations. The nominated individual has not followed up with the local authority designated officer (LADO) regarding allegations that were made against staff which led to the first monitoring visit on 11 September 2025.

p.2

This monitoring visit

Before the restriction notice was issued, children with complex needs and high levels of risk were admitted to the service. There had been a failure to carry out thorough assessments of risk to ensure that children’s needs can be appropriately met in supported accommodation. This failure has placed children at risk of harm. Leaders and managers did not have effective procedures to show how they will reduce risks for children moving into the service.

p.2

This monitoring visit

Progress in addressing the significant shortfalls identified at the last two monitoring visits has been slow. Leaders and managers have failed to demonstrate that they are providing sufficient care, competence and skill. Compliance notices are not met. Additional shortfalls and further concerns have been identified as a result of this visit.

p.2

This monitoring visit

Safer recruitment practices are not in place. Staff working with children do not have suitable Disclosure and Barring Service background checks. There is insufficient evidence that pre-employment references had been obtained. Reasons for staff leaving previous employment and gaps in employment have not been explored.

p.2

This monitoring visit

Ofsted considers that admitting further children to the service would place them at significant risk of harm. A notice restricting the accommodation of any new children remains in place.
