p.1

This home is owned by a private provider and is registered to provide care for up to three children who may require support with social and emotional needs.
p.1
Information about this children’s home
The registered managers post has been vacant since September 2024. There is an interim home manager in post who has not yet registered with Ofsted.
p.1
Information about this children’s home
The home manager is absent from work. Another interim home manager is supporting during this period. This interim home manager and responsible individual were present during this visit.
p.1
Information about this children’s home
There were two children living at the home at the time of the visit, and their views were sought.
p.1
This monitoring visit
This home was found to have serious and widespread concerns at the assurance inspection on 20 October 2025. Following the inspection, two statutory requirements were set, and two compliance notices were issued in relation to regulation 12, the protection of children standard and regulation 13, the leadership and management standard.
p.1
This monitoring visit
Ofsted issued a restriction of accommodation notice to ensure that no further children were admitted given the identified concerns.
p.1
This monitoring visit
This visit was carried out to evaluate whether the actions set following the assurance inspection in relation to the protection of children and leadership and management have been completed. Inspection report for children’s home - monitoring visit: 2777336 2 There has been no registered manager in post since September 2024. Since this date, there have been three home managers appointed, who have not registered with Ofsted. At the time of the visit, the home’s manager had been absent for a period of over a week. Another interim home manager is supporting during this period.
p.1
This monitoring visit
Risk management plans do not identify all known risks for children, leaving significant gaps in the strategies required to keep children safe. Behaviour support plans do not match with individual risk assessments for the children. This inconsistency means that staff are unclear about the measures they should take to reduce risk or respond safely when incidents occur.
p.2
This monitoring visit
The arrangements for the supervision of children are ineffective. Staff do not receive consistent guidance about agreed supervision levels for each child to be supported across different areas of the home. The use of door alarms lacks appropriate oversight and door alarms are activated throughout day and night without sufficient review of their usage. In one instance, a child’s social worker reports not having consented to the ongoing use of permanent door alarm monitoring and was unclear what level of monitoring was in place.
p.2
This monitoring visit
Incident records, including those involving physical intervention, are inadequate and do not meet regulatory requirements. One incident of physical intervention has not been recorded correctly. Despite reviewing the incident, the manager failed to record any action taken to address the inaccurate recording and did not evaluate the use of physical intervention. As a result, the safeguarding network cannot be assured that the physical hold used was appropriate, necessary or proportionate.
p.2
This monitoring visit
An incident involving an allegation against a staff member was not managed appropriately. The allegation has not been investigated as the manager has not spoken to the child or the staff member. There has been no liaison with the local authority designated officer, and the incident has not been notified to Ofsted. Therefore, no action has been taken to safeguard the child following an allegation of harm.
p.2
This monitoring visit
The management of complaints is ineffective and does not protect children. An external complaint has not been managed appropriately and there has been an avoidable delay in initiating a review due to the manager’s absence from the home. Although the interim manager has spoken with the complainant, they have failed to gather the essential information required to carry out an investigation. Crucially, the date of the alleged incident has not even been established. As a result, no relevant recordings have been reviewed and neither the child nor the staff involved have been spoken to. As a result, leaders and managers cannot be assured that the child was safe during the alleged incident.
p.2
This monitoring visit
Staff training and development has been reviewed in relation to managing a child’s health risks. A medical protocol has been developed, and learning has been taken Inspection report for children’s home - monitoring visit: 2777336 3 from incidents and shared with the wider staff team to ensure that staff respond consistently to the child’s health needs.
p.2
This monitoring visit
Processes for assessing the suitability for children to live together have been reviewed and strengthened. However, as there is a restriction of accommodation in place there are no new children. Therefore, the revised procedures have not yet been implemented or evaluated for effectiveness.
p.3
This monitoring visit
Management oversight is ineffective. Although the interim home manager has completed an audit of incidents, they have failed to address poor staff practice or inaccuracies in recording. Furthermore, oversight from the responsible individual is also insufficient. Despite having oversight of the audits and managers responses, the responsible individual did not recognise or act on clear shortfalls in practice and recording. The responsible individual has failed to effectively supervise the management team. As a result, no learning has been taken from incidents and opportunities to improve staff practice have been missed.
p.3
This monitoring visit
The two compliance notices and statutory requirements are considered not met therefore the restriction of accommodation notice remains in place. Inspection report for children’s home - monitoring visit: 2777336 4
