p.1

provider

At the time of this visit, the service was providing supported accommodation for eight looked-after children and care leavers aged 16 and 17.

p.1

provider

The service manager registered with Ofsted in April 2025. This visit was facilitated by the nominated individual, who is also the director.

p.1

This monitoring visit

Ofsted carried out a full inspection on 1 September 2025. The inspection identified serious shortfalls affecting the experiences and progress of children. Ofsted issued two compliance notices relating to leadership and management and the suitability of staff.

p.1

This monitoring visit

A monitoring visit on 15 October 2025 found improvements in the leadership and management arrangements. The visit found that the provider had taken action to address some of the concerns raised in the compliance notices. Sufficient action has been taken in respect of the compliance notice relating to leadership and management. However, shortfalls in safer recruitment practice remain. The provider has not demonstrated that they have a system to ensure that safer recruitment practices are followed. The managers were unable to demonstrate that all staff recruitment files include full and satisfactory information. Therefore, a compliance notice relating to the suitability of staff was issued. Inspection report for supported accommodation provider - monitoring visit: On Track Care Services LTD This visit was carried out to monitor the provider’s progress in meeting the steps identified in the compliance notice relating to the suitability of staff.

p.1

This monitoring visit

The visit found that the provider has taken sufficient action to address the steps set out in the compliance notice.

p.2

This monitoring visit

The provider has reviewed all recruitment files. This includes staff who were appointed several years ago. The provider has followed up on gaps in employment histories and sought explanations.

p.2

This monitoring visit

The provider has improved the recording system. All the information was easily accessible during this visit.

p.2

This monitoring visit

The provider has introduced changes to the application form for future use. This increases the potential for clear, full employment histories to be provided from the outset and for gaps to be explored. The changes also mean that the applicant will need to provide contact details for someone at all previous jobs. This will increase the ease with which the provider could follow up on reasons why people have left previous roles with children or vulnerable adults.

p.2

This monitoring visit

The provider has modified the reference form to capture clearer information. The provider has demonstrated effective follow-up by verifying a ‘basic’ reference.

p.2

This monitoring visit

All files sampled included Disclosure and Barring Service checks. The provider has also sought overseas checks when necessary.
