Inspection report for children’s home - monitoring visit: 2677465 1 Registered person: Madiba Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

This home is owned and managed by a private provider. It provides care for up to four children who experience social and emotional difficulties, and who may have learning disabilities.

p.1

Information about this children’s home

There has been no registered manager at the home since 21 August 2025. A manager has been recruited but is still to start working at the home.

p.1

Information about this children’s home

At the time of this visit, no children were living at the home.

p.1

This monitoring visit

On 15 and 16 July 2025, a full inspection was completed. Serious and widespread concerns were found relating to the safeguarding of children and the leadership and management of the home. A suspension notice was issued together with two compliance notices.

p.1

This monitoring visit

On 2 September 2025, a monitoring visit was completed. This was to monitor the action taken by the provider to meet the steps within the two compliance notices issued after the full inspection. There was no staff team in place and no manager in post. The provider had not met the steps within the compliance notices, so a further notice to suspend the home’s registration was issued.

p.1

This monitoring visit

This monitoring visit was carried out to review the provider’s compliance with the suspension notice. No children have moved into the home, so the provider has not breached the notice. Inspection report for children’s home - monitoring visit: 2677465 2 A new manager has been appointed and there is a plan for him to apply to register with Ofsted. There is a staff team in place, with sufficient staff to provide care for one child. Recruitment for additional staff is ongoing.

p.1

This monitoring visit

Ofsted has received an application to change the responsible individual. The provider has made further changes to its organisational structure at senior management level. This includes the recruitment of managers with responsibility for reviewing and monitoring the quality of care provided to children, as well as reviewing staff development and practice.

p.2

This monitoring visit

Senior leaders have improved the systems in place to ensure that children’s risk assessments provide staff with clear guidance, and for these to be kept under review by the manager.

p.2

This monitoring visit

Senior leaders and managers now attend regular group meetings to discuss safeguarding concerns and staff conduct across the organisation. A new system has been implemented to track and monitor steps taken to address the concerns and the outcome of any investigations. This tool is effective and improves management oversight. There is now a collaborative approach to decision-making about safeguarding concerns and allegation management, with stronger oversight by senior leaders.

p.2

This monitoring visit

Leaders have reflected on the shortfalls identified at the full inspection. There is a plan in place for the clinical psychology team to complete a skills analysis of all staff. This will determine any additional learning and development needs. The clinical team has delivered training to those responsible for the supervision of staff. This is to equip supervisors with the skills to provide staff with supervision that is reflective and focuses on staff’s safeguarding knowledge.
