p.1
Inspected under the social care common inspection framework
p.1
provider
On Track Care Services LTD is a private provider, offering ring-fenced shared accommodation for up to 16 children across three premises in West London.
p.1
provider
At the time of the monitoring visit, the service was providing supported accommodation for eight children aged 16 or 17 who were looked-after children or care leavers.
p.1
provider
The service registered with Ofsted in January 2024. The current registered service manager registered in April 2025. The monitoring visit was facilitated by the nominated individual, who is also the director.
p.1
This monitoring visit
Ofsted carried out a full inspection on 1 September 2025. The inspection identified serious shortfalls impacting on the experiences and progress of children. Ofsted issued two compliance notices relating to leadership and management and the fitness of staff.
p.1
This monitoring visit
This monitoring visit was carried out to monitor the provider’s progress in meeting the steps identified in the compliance notices.
p.1
This monitoring visit
The inspection found that the provider has taken action to address some of the concerns raised in the compliance notices. Sufficient action has been taken in respect of the compliance notice relating to leadership and management. However, shortfalls in safer recruitment practice remain. The provider has not demonstrated that they have a system to ensure that safe recruitment practices are followed. The managers were unable to demonstrate all staff recruitment files had full and Inspection report for supported accommodation provider - monitoring visit: On Track Care Services LTD satisfactory information available. Therefore, a compliance notice relating to the fitness of staff was issued.
p.1
This monitoring visit
No children have been admitted since the inspection. Only children within the scope of supported accommodation and recently turned 18 care leavers were accommodated at the time of the monitoring visit.
p.2
This monitoring visit
The nominated individual and the service manager have introduced new procedures to help evaluate the suitability of new admissions. Changes include ensuring that there is sufficient information at the point of referral to determine the risks and needs of children and consider the impact on children already living in the homes.
p.2
This monitoring visit
Leaders have introduced weekly staff meetings to improve the communication with staff. This increases the potential for a shared understanding of the children’s needs and better preparation for new admissions.
p.2
This monitoring visit
The provider has begun to explore, but not yet addressed, issues relating to a blanket approach to risks, such as removing knives from the kitchen, storing medication in the office in the homes and the use of CCTV. These approaches do not help prepare children for independent living.
p.2
This monitoring visit
The nominated individual has extended access to staff recruitment files to junior managers to try to ensure consistency during the absence of senior managers. Consequently, staff files were accessible during the monitoring visit.
p.2
This monitoring visit
There are weaknesses in the recruitment process, particularly in exploring staff’s employment history, gaps in employment and references, and reasons for leaving previous employment with children or vulnerable adults. Some application forms were missing dates of employment, and there was no evidence that the provider explored information that should have prompted curiosity.
p.2
This monitoring visit
The provider does not have a clear overview of staff training needs. Not all staff have up-to-date mandatory training, and one member of staff was not on the training matrix at all. There is no clear indication of how induction and probation periods are evaluated, and what additional support staff who have limited experience with working with children in care and/or are new to the country may need in order to adjust to their new roles. This potentially limits the support that staff can provide to the children.
p.2
This monitoring visit
Leaders and managers have ensured that following a recent incident at one accommodation, timely action was being taken to carry out the necessary repairs and support the children. Improvements have also been made to the quality of some accommodation.
p.2
This monitoring visit
Leaders and managers have introduced new templates to record support plans. These are tailored towards children’s individual needs and goals. Staff were updating these appropriately with relevant information during the monitoring visit. Inspection report for supported accommodation provider - monitoring visit: On Track Care Services LTD The other requirements raised outside of the previous compliance notices were not reviewed as part of this monitoring visit. Therefore, they are restated and will be reviewed as part of the next inspection. Inspection report for supported accommodation provider - monitoring visit: On Track Care Services LTD
