The home is privately operated and provides care for one child who may experience social and emotional difficulties.
p.1
Information about this children’s home
At the time of this inspection, the setting was suspended, and a notice to cancel the provider’s registration had been issued.
p.1
Information about this children’s home
The home and the manager registered with Ofsted in June 2025.
p.1
This monitoring visit
A monitoring visit was carried out on 27 August 2025 that identified serious and widespread concerns. The visit was carried out to review the actions taken by the provider following a decision to cancel the provider’s registration.
p.1
This monitoring visit
Inspectors considered the provider’s action plan to determine whether sufficient action has been taken to demonstrate that the provider can safely care for children and have the suspension lifted.
p.1
This monitoring visit
Key policies, such as those relating to children going missing from home, do not align with local safeguarding procedures. This creates ambiguity for staff and risks inconsistent responses in critical situations. Staff do not have a clear understanding of what is expected of them, which undermines the effectiveness of safeguarding arrangements.
p.1
This monitoring visit
The manager has carried out a lessons-learned exercise with the staff team. However, it lacked meaningful reflection on what went wrong and failed to consider the individual practice of staff, including leaders and managers. There was no exploration of accountability or how future incidents could be prevented. Inspection report for children’s home - monitoring visit: 2835212 2 The provider has placed a strong emphasis on delivering training to staff in areas such as ligature, self-harm, and children going missing from home. This has been beneficial in improving staff’s awareness and response in specific scenarios. However, most of this training has been delivered internally by the manager, whose leadership and oversight were previously associated with repeated incidents of significant harm to a child living at the home. This raises concerns about the appropriateness and credibility of the training, particularly in the absence of any

This monitoring visit
evaluation of the manager’s capability to deliver it effectively.
p.2
This monitoring visit
There has been no meaningful scrutiny of the manager’s leadership or decision- making following the suspension. The only supervision session held with the manager focused solely on staff training, missing a critical opportunity to assess his competence, accountability, and ability to manage risk. There is no evidence that the provider has explored the manager’s role in previous failings or considered whether he is equipped to lead the service safely moving forward.
p.2
This monitoring visit
Despite multiple opportunities to identify and address risk, the manager and the responsible individual failed to do so. There is no evidence of either recognising shortfalls in staff practice or considering performance issues arising from serious safeguarding incidents. The absence of individual accountability and reflective learning continues to raise serious concerns about the provider’s capacity to protect children from harm and to lead a safe and effective service.
