Inspection report for supported accommodation provider - monitoring visit: Healthfield House 1 Healthfield House

p.1

Registered provider: Healthfield Medical Services Ltd

p.1

Inspected under the social care common inspection framework

p.1

provider

The service is owned and managed by a private provider. It offers two group-living premises that are not limited to accommodating looked-after children or care leavers.

p.1

provider

At the time of this visit, the service was providing accommodation for two children aged 16 and 17.

p.1

provider

There is no registered service manager at the service. The nominated individual was registered with Ofsted when the service was registered in June 2025.

p.1

This monitoring visit

A monitoring visit was undertaken on 17 September 2025 after Ofsted was made aware that a serious incident had occurred at this undertaking on 12 September 2025. This incident is currently under investigation by the appropriate authorities. While Ofsted does not have the power to investigate incidents of this nature, the actions taken by the setting in response to the incident were considered alongside other evidence available at the time of the visit.

p.1

This monitoring visit

During the visit, social care regulatory inspectors identified significant regulatory breaches relating to the protection standard and a failure to place and support children in line with the requirements of supported accommodation.

p.1

This monitoring visit

Leaders and managers fail to provide a homely living environment to children, ensuring that it is registered with Ofsted. Findings indicate that one of the premises has an annexe providing single-occupancy accommodation, meaning the provider is operating an unregistered setting. The annexe contains litter and is very untidy. Another child’s bedroom is inadequately furnished, as it contains only a mattress on Inspection report for supported accommodation provider - monitoring visit: Healthfield House 2 the floor without a bed frame. Additionally, a fridge is located in the hallway instead of the child’s bedroom.

p.1

This monitoring visit

Serious weaknesses in decision-making by leaders and managers have left children at significant risk of harm. Leaders and managers admit children into the homes who require high levels of care and monitoring. This is not consistent with the conditions of registration for supported accommodation. Leaders and managers have failed to recognise this.

p.2

This monitoring visit

Despite children’s placing local authorities providing clear referral information indicating a high level of need, and parents sharing concerns about their children, managers fail to take this information into account. This has resulted in serious harm to children and unplanned placement endings.

p.2

This monitoring visit

Leaders and managers have implemented assessment processes and plans to guide staff practice in supporting children. However, these assessments have failed to identify children’s risks effectively or guide staff appropriately.

p.2

This monitoring visit

Children are being supported and cared for by untrained staff. Leaders and managers have failed to ensure that all staff possess the necessary knowledge, understanding and skills to meet the complex needs of the children.

p.2

This monitoring visit

There is no registered service manager in post. Although a service manager has been appointed, they have yet to demonstrate to Ofsted that they meet the requirements for the role. The service manager has been involved in decisions to accept children whose needs fall outside the scope of the service, indicating a lack of understanding of the remit of supported accommodation.

p.2

This monitoring visit

Recruitment processes fail to reflect the diversity needed in staff teams to meet the needs of children from different cultural backgrounds. Language barriers and cultural differences between staff and children have led to communication failures and an inability to address children’s concerns and needs effectively.

p.2

This monitoring visit

Leaders and managers have failed to report some serious incidents, including high- level behaviours exhibited by children that could have resulted in serious harm. As a result, Ofsted has been unable to review the provider’s actions and responses.

p.2

This monitoring visit

Leaders and managers do not have effective monitoring and reporting systems in place. They have failed to complete and share their six-monthly support review with Ofsted. This prevents the regulator from assessing the actions being taken by the provider as a result of the review.
