p.1

This monitoring visit

This visit was carried out following information received by Ofsted indicating that children may be at risk of harm due to significant concerns. The provider was given notice on the morning of the visit. The visit took place at the location where a child, identified as being at high risk, was residing. The child in question was spoken to during the visit.

p.1

This monitoring visit

Leaders and managers fail to work collaboratively with, or challenge, children’s local authorities in sourcing suitable placements. This increased the risk of harm to children, led to a rise in staffing levels, and necessitated high levels of monitoring to ensure safety.

p.1

This monitoring visit

Children have been involved in several serious and high-risk incidents, both at home and in the community. Leaders and managers responded to these risks by ensuring that staff received training aligned with the children’s current needs. However, some actions taken, such as increasing staffing levels during both day and night, were not Inspection report for supported accommodation provider - monitoring visit: Youth Transitioning Support Ltd in line with the requirements of a supported accommodation provision. This would suggest that some of the children were not ready for supported accommodation.

p.1

This monitoring visit

Staff have not always managed incidents in the home effectively when children have been distressed. This has meant that staff have allowed children living in one home to see and hear distressing incidents. Children have observed emergency services attending the premises to provide medical support, and they have witnessed the management of difficult behaviours in communal areas.

p.2

This monitoring visit

Leaders and managers fail to carry out effective pre-admission assessments to ensure that children are ready for supported living and are compatible with other children living in the home. Despite children’s local authorities providing clear referral information indicating a high level of need, managers failed to take note of this information. This resulted in serious harm to children and unplanned placement endings.

p.2

This monitoring visit

Children’s support plans outline current risks. One plan directs staff to remain highly vigilant and closely monitor a child who is at higher risk. Support staff produce incident reports that outline the actions taken during incidents to manage children’s behaviours and ensure their safety. However, the reports lack managerial oversight.

p.2
