Inspection report for children’s home - monitoring visit: 2730253 1 Registered provider: Madiba Yorkshire Limited Monitoring visit Inspected under the social care common inspection framework

p.1

Information about this children’s home

This home is owned and operated by a private provider. It offers care for up to three children who experience social and emotional difficulties and may have learning disabilities.

p.1

Information about this children’s home

At the time of this visit, two children were living at the home.

p.1

Information about this children’s home

The manager registered with Ofsted in November 2024. The registered manager was not working at the home at the time of the visit.

p.1

This monitoring visit

An assurance inspection was carried out on 23 July 2025. During this inspection, Ofsted identified significant shortfalls in the leadership and management of the home. As a result of the shortfalls, a decision was made to issue a notice restricting accommodation to prevent children moving into the home. Ofsted also issued a compliance notice under regulation 13. In addition, several additional requirements were raised.

p.1

This monitoring visit

This visit was carried out to monitor the provider’s compliance with the notice restricting accommodation and its progress in meeting the steps set out in the compliance notice.

p.1

This monitoring visit

The provider has complied with the notice restricting accommodation, and no children have moved into the home.

p.1

This monitoring visit

The provider has made progress in meeting some of the steps in the compliance notice. The unmet steps have been restated. Inspection report for children’s home - monitoring visit: 2730253 2 The children’s risk assessments and care plans have been reviewed. These are now clearer about the levels of risk affecting children and what staff need to do to safeguard them. The acting manager has implemented a new system so that social workers can review children’s risk assessments and care plans. Furthermore, a new process is in place to ensure that all staff working with children have read and are familiar with children’s plans.

p.1

This monitoring visit

Additional training has been identified to support staff in meeting children’s specific needs, including responding to self-harm and using ligature cutters. Most of the core staff team has completed the training, and there are plans for the remaining staff to complete this in the coming weeks. The staffing arrangements have been reviewed to ensure that there is always a trained member of staff available.

p.2

This monitoring visit

Since the last inspection, the registered manager has stopped working for the provider and has requested to cancel their registration voluntarily. The provider has appointed an interim manager to work in the home and has already started the process of appointing a new manager who intends to register with Ofsted.

p.2

This monitoring visit

Senior leaders have increased their monitoring of the home. This includes being visibly present in the home to support the interim manager and deputy manager and to provide assurances to the children.

p.2

This monitoring visit

New monitoring systems have been developed. However, some of these are yet to be used, and others are in their infancy. The effectiveness of these new systems is therefore untested.

p.2

This monitoring visit

The children continue to receive inconsistent care due to unfamiliar staff working in the home. There is still a heavy reliance on staff support from the provider’s other homes, along with some use of agency staff.

p.2

This monitoring visit

During the visit, a significant shortfall was identified in the vetting of an agency member of staff. This left children at risk of harm. The operations manager took immediate action to prevent reoccurrence and provided assurances to Ofsted.

p.2

This monitoring visit

The requirements previously raised under regulations 11, 12, 13, 33, 35 and 40 were not reviewed during this visit. For this reason, they are restated. An additional requirement was raised under regulation 32. Inspection report for children’s home - monitoring visit: 2730253 3
