Information about this inspection
the care of children the safety of children the effectiveness of leaders and managers. The inspector has looked closely at the experiences and progress of children, using the social care common inspection framework. This assurance inspection was carried out under the Care Standards Act 2000 to assess the effectiveness of the service, how it meets the core functions of the service as set out in legislation, and to consider how well it complies with The Children’s Homes (England) Regulations 2015 and the ‘Guide to the Children’s Homes Regulations, including the quality standards’. Assurance inspection report for children’s home: 2775374 2
p.1
Findings from the inspection
We did not identify any serious or widespread concerns in relation to the care or protection of children at this assurance inspection.
p.2
Findings from the inspection
The child has made progress with their emotional health. The child is physically healthy and their confidence has noticeably improved. This positive transformation indicates improved feelings of safety because of the care provided by staff.
p.2
Findings from the inspection
In recognition of the child’s progress, the provider’s clinical team proposes a review of the child’s diagnosis, a reduction of staffing levels required to keep the child safe, and changes to restrictions made by the courts to keep the child safe. The clinical team advocates that the child can now live safely with other children.
p.2
Findings from the inspection
The child’s progress reflects the collaborative efforts between staff and the clinical team to implement support strategies consistently and effectively. The child has actively contributed to the creation of their own safety plan, which includes personalised approaches for receiving support during periods of emotional distress.
p.2
Findings from the inspection
The child’s behaviour support plan and risk assessment are reviewed and updated following incidents. There has been a notable decrease in ligature-related incidents, with none reported since the last full inspection. However, there is an emerging risk of self-injury through biting, which is not currently reflected in the child’s risk assessment as increasing.
p.2
Findings from the inspection
Education and learning are promoted well. The child has a positive attitude to school and learning. They express aspirations for the future. This is notable progress for this child and is reflective of the sense of safety they have developed.
p.2
Findings from the inspection
The child is held regularly, with some holds lasting several minutes. On one occasion, staff used a prolonged hold to keep the child safe. However, the manager does not comprehensively evaluate the appropriateness of the types of holds being used. This poses a risk and limits the manager’s oversight of holds that may further distress the child.
p.2
Findings from the inspection
One member of staff carried ligature cutters before the manager had ensured that they had the required training. Furthermore, three agency workers do not have ligature training. Therefore, the manager has not ensured that all staff caring for children are skilled in line with the reassurances the manager provided to Ofsted at the last inspection. Notably, these staff members are scheduled to work during times when the child is most vulnerable to self-injurious behaviour.
p.2
Findings from the inspection
There has been a reduction in ligature-related incidents. However, other forms of self-injurious behaviour remain. Staff responded appropriately to one incident of distress to prevent further harm and administered first aid appropriately. However, leaders and managers have not provided a specific self-injury protocol for staff to Assurance inspection report for children’s home: 2775374 3 follow. Therefore, staff were unclear about seeking medical support from a qualified health professional in response to the self-injurious incident. Most importantly, the shortfall in consideration of the seriousness of such incidents means that the regulator has been unable to assess the effectiveness of risk management in the home.
p.2
Findings from the inspection
The manager continues to rely on agency staff for waking night duties due to ongoing recruitment challenges. Six agency workers are used, all of whom are familiar to the child and have been working regularly. While there is evidence of safer recruitment practices, these workers do not receive practice-related supervision at the home.
p.3
Findings from the inspection
Furthermore, staff practice-related supervisions are not up to date. There is inconsistency in how new members of staff are supervised and some staff are unaware of their schedules for supervision. This lack of structured oversight by leaders and managers raises concerns about the consistency and reliability of support and guidance provided to staff, particularly when the child has complex and high-risk behaviours.
p.3
Findings from the inspection
The leadership of the home is not strong. The deputy manager and the responsible individual have resigned from their respective posts. One of the directors has assumed the role of responsible individual and will be providing increased oversight and support to strengthen the manager’s leadership of the team.
p.3
Findings from the inspection
Leaders and managers provided reassurances and are working towards the action plan they formulated, which is not yet fully met. One requirement has been removed. No new requirements or recommendations have been made at this assurance inspection. Assurance inspection report for children’s home: 2775374 4
