Inspection report for children’s home - monitoring visit: 2557064 1 Registered provider: A-Triangle Care Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

This home is operated by a private company. It provides care for up to two children who experience social and emotional difficulties and learning difficulties.

p.1

Information about this children’s home

The home was registered with Ofsted in August 2020. The registered manager post and the responsible individual post are vacant.

p.1

Information about this children’s home

At the time of the visit, the home was subject to a suspension of accommodation notice and there were no children living in the home.

p.1

This monitoring visit

A full inspection was undertaken on 17 April 2025. During this visit, inspectors identified serious and widespread concerns in relation to the care and protection of children and the leadership and management of the home.

p.1

This monitoring visit

Following the full inspection, Ofsted made a decision to suspend the accommodation at the children’s home under section 14A of the Care Standards Act 2000 (the Act) with immediate effect.

p.1

This monitoring visit

This monitoring visit was carried out to monitor the suspension of accommodation notice. Inspectors found that the provider has complied with the suspension of accommodation notice. However, suitable action to address all shortfalls identified at the inspection had not been taken.

p.1

This monitoring visit

Since the inspection on 17 April 2025, a manager has been appointed who has submitted an application to register with Ofsted. However, the new manager has not yet been in post long enough to affect significant change or drive measurable improvements. Inspection report for children’s home - monitoring visit: 2557064 2 Staff recruitment processes continue to be unsafe, with key safer recruitment checks not being completed for staff who are new to the home. Once staff member did not have sufficient Disclosure and Barring Service checks completed prior to their employment and these shortfalls were not identified by the provider.

p.1

This monitoring visit

Leaders and managers have created an action plan following the inspection. However, they were unable to provide sufficient evidence to demonstrate how these actions had been met.

p.2

This monitoring visit

Leaders and managers say that they have held regular team meetings with staff to discuss shortfalls and support staff development. However, meeting minutes do not reflect these discussions, and reflection sessions are not evident.

p.2

This monitoring visit

Staff who continue to work at the home have not received supervision since the home was suspended. This means that staff have not had the opportunity to reflect on the inspection and identify lessons learned from the shortfalls identified.

p.2

This monitoring visit

Some staff have received training sessions since the last inspection to support their development and improve their skills and knowledge around de-escalation techniques and recording and reporting. However, staff have not received refresher training on safeguarding and managing allegations. This means that staff have not considered the expectations of their roles and responsibilities in response to safeguarding concerns.

p.2

This monitoring visit

External monitoring visits of the home continue to be ineffective in providing external scrutiny. Two reports completed since the last inspection include inaccurate information and do not evaluate the action taken during the reporting periods.

p.2

This monitoring visit

The suspension of accommodation notice will remain in place. The requirements regarding areas of concern raised at the full inspection also remain in place and will be reviewed at the home’s next inspection. Inspection report for children’s home - monitoring visit: 2557064 3
