Inspection report for supported accommodation provider - monitoring visit: Agile Care Solutions Ltd 1 Agile Care Solutions Ltd
p.1
Registered provider: Agile Care Solutions Ltd
p.1
Inspected under the social care common inspection framework
p.1
provider
This supported accommodation service is owned and managed by a private provider. The service provides two shared supported accommodation premises. One is limited to accommodating looked-after children and care leavers, and the other is not limited to accommodating care leavers.
p.1
provider
At the time of this monitoring visit, this service provided supported accommodation for three children aged 17 years old. The registered service manager resigned in December 2024 and has not been replaced. The nominated individual and director were registered with Ofsted when the service was registered in October 2024.
p.1
This monitoring visit
This monitoring visit was carried out due to Ofsted receiving information that children may be at risk of harm due to significant concerns. The provider was not given notice of the visit. The monitoring visit took place at the only location operated by the provider where children were living. Two children were spoken with during the visit.
p.1
This monitoring visit
Children have been involved in several serious and high-risk incidents in the community, with at least one experiencing violence and exploitation. Leaders and managers had not responded to risk effectively or swiftly. Staff were unable to view risk assessments or safety plans related to the highest risk incidents, leaving children open to repeated risk of harm.
p.1
This monitoring visit
Leaders and managers fail to ensure children’s safety and are unable to recognise when children have placed themselves at risk of harm and need involvement of other professionals. The provider failed to notify Ofsted of two incidents that were serious events, as required by legislation. This has prevented Ofsted from having an Inspection report for supported accommodation provider - monitoring visit: Agile Care Solutions Ltd 2 accurate understanding of how the service keeps children safe and from carrying out its regulatory functions. On another occasion, the provider failed to inform the child’s local authority of a serious incident. This prevents the child’s social worker from having an accurate picture of the child’s current risks and experiences and being able to understand if the current plan meets the child’s needs.
p.1
This monitoring visit
Serious weaknesses in leadership have left children at significant risk of harm. Leaders and managers have admitted one child to the setting who is in receipt of care and who requires a high level of supervision. This is not consistent with the conditions of registration for supported accommodation. Leaders, managers and staff did not recognise this.
p.2
This monitoring visit
Staff do not feel well supported by the organisation. Staff describe working excessive hours and many days consecutively, including during illness. Staff had not been made aware of one child’s commissioned hours, so were not providing the level of supervision that the child needed to remain safe.
