p.1
This monitoring visit
The purpose of this monitoring visit was to review the actions taken by the provider to meet the compliance notices issued in relation to regulations 12 and 13. Ofsted found that the provider had not taken sufficient action to meet the compliance notices. Inspection report for children’s home - monitoring visit: 2689909 2 Since the last visit, staff have not taken action to ensure that the child has been safeguarded from peers in the community. Staff did not follow plans to prevent the child from attending the home of a peer. This led to an incident of the peer visiting the home. Staff did not intervene to prevent the child from being exposed to verbal abuse and threats.
p.1
This monitoring visit
Staff and managers do not recognise or understand the child’s risks of sexually inappropriate behaviour. Information in the child’s risk assessment documents attributes the child accessing pornographic adult content online as expected for children exploring their sexuality.
p.2
This monitoring visit
Staff and managers did not recognise the risks of the child having access to an internet-enabled television in their bedroom. As a result, the child has continued to have access to electronic devices, including a mobile phone, on an unsupervised basis. Action has not been taken following incidents of the child accessing inappropriate content and websites online via their television. Furthermore, action has not been taken following the child’s inappropriate use of their mobile phone.
p.2
This monitoring visit
Plans for the child do not include clear guidance for staff to follow to manage the risks of the child harming themselves. Clear strategies are not identified in the staff guidance records. Therefore, staff do not have access to clear guidance on how to respond and safeguard the child if incidents occur.
p.2
This monitoring visit
Opportunities for the staff team to undertake training to meet the child’s needs had not been provided at the time of the visit. Furthermore, additional training identified for one member of staff following an internal investigation of their practice has not taken place.
p.2
This monitoring visit
The manager has not identified shortfalls in the staff’s recording of incidents. The manager has not identified that incidents have been inaccurately recorded in the child’s records. Plans for the child do not include all relevant information following incidents that have occurred. Furthermore, delays in the managerial evaluation of incidents means that the manager has not taken prompt action to address shortfalls in staff practice.
p.2
This monitoring visit
This monitoring visit identified further shortfalls in safeguarding practices and the leadership and management of the home.
p.2
