p.1
provider
This service is owned and managed by a private provider. The service provides shared accommodation that is not limited to looked-after children and care leavers. Up to two children or young people can live at the premises.
p.1
provider
At the time of this visit, one child was living at the premises.
p.1
provider
The registered service manager, who is also the nominated individual, has been in post since the service registered in September 2024.
p.1
This monitoring visit
Ofsted carried out this visit after the child’s placing authority shared concerns that it identified as part of its own quality assurance visit. The information raised concerns about the provider’s ability to provide effective support that keeps the child and others safe.
p.1
This monitoring visit
The manager has failed to make suitable arrangements to ensure the safety of the child or staff. The child’s needs, risks and behaviours are not captured in written plans or risk assessments. This means that staff do not have key information that enables them to manage known and emerging risks in a planned way. This includes an absence of work in helping the child to understand and reduce risk-taking behaviour. In addition, staff have not received training in child exploitation, mental health, or substance misuse. Therefore, responses to these types of behaviour have not been effective. This, combined with a lack of management oversight and Inspection report for supported accommodation provider - monitoring visit: A G Semi-Independent Accommodation understanding of risk, has resulted in the child, staff and others being placed at potential risk of harm.
p.1
This monitoring visit
The manager has failed to notify Ofsted and the local authority when serious incidents involving the child have occurred. Due to an absence of records, it is difficult to verify what actions have been taken to manage these incidents, many of which have involved a child who previously lived at the premises.
p.2
This monitoring visit
Systems for maintaining private and confidential records are inadequate, with no secure location for these in the home. Very few records were available for review at the time of the visit. This was because the child and a friend had reportedly accessed and destroyed several records. Consequently, key information relating to the child’s experiences has been lost.
p.2
This monitoring visit
Management oversight is weak and has not ensured that the child and staff have been given the support they require. The manager has failed to investigate or respond to a significant concern raised by a member of staff. This has potentially compromised the safety of the child and staff. Staff supervision is not purposeful and is not used to help staff to reflect, improve their practice or manage risk.
p.2
This monitoring visit
Following this visit, a notice restricting accommodation, to prevent new admissions, was issued. The placing authority for the child has put in place additional support measures, including additional staff and systems for checking and monitoring the implementation of the child’s support plan.
