Inspection report for supported accommodation - monitoring visit: 2759620 1 Registered provider: Key Transitions Limited

p.1

Inspected under the social care common inspection framework

p.1

provider

This service is owned and operated by a private provider. The service can care for up to seven children living in solo accommodation and up to 51 children living in 13 shared premises that are limited to looked after children and care leavers.

p.1

provider

The registered service manager has been in post since the service registered in December 2023.

p.1

This monitoring visit

Ofsted carried out this monitoring visit in response to whistle-blowing concerns about staff’s ability to meet children’s individual needs and support them to stay safe. Concerns were also raised about management oversight and support, allegations management and children’s records.

p.1

This monitoring visit

Children’s needs and risks are outlined in their written plans. However, these documents do not always include information about how children’s known and emerging risks are to be managed by staff. Furthermore, there is no evidence that staff are completing planned work to help children understand and reduce their risk- taking behaviour. Although children are generally well cared for, this does not support a planned approach to meeting children’s needs or keeping them safe.

p.1

This monitoring visit

Management oversight requires improvement. Systems are in place to ensure that members of the management team regularly visit each premises to assure themselves that children’s needs are being met in line with their plans and to share pertinent information with the registered service manager. These systems are not wholly effective. For example, the registered service manager had not been made Inspection report for supported accommodation - monitoring visit: 2759620 2 aware of some serious incidents involving children. This has also resulted in a failure to keep Ofsted informed when serious incidents have occurred.

p.1

This monitoring visit

Allegations against staff have been suitably managed. Action is taken to mitigate the immediate risk, and information is shared with external safeguarding partners. This ensures that children are protected.

p.2

This monitoring visit

Children’s information is not always managed in a way that maintains their confidentiality. For example, staff have used their personal electronic devices to record, store and share information about children and to manage children’s finances. This does not protect children or staff.
