p.1

Information about this inspection

 the care of children  the safety of children  the effectiveness of leaders and managers. Inspectors have looked closely at the experiences and progress of children, using the social care common inspection framework. This assurance inspection was carried out under the Care Standards Act 2000 to assess the effectiveness of the service, how it meets the core functions of the service as set out in legislation, and to consider how well it complies with The Children’s Homes (England) Regulations 2015 and the ‘Guide to the Children’s Homes Regulations, including the quality standards’. Assurance inspection report for children’s home: 2633744 2

p.1

Outcome from the inspection

We identified the following serious and widespread concerns in relation to the care or protection of children at this assurance inspection:

p.2

Outcome from the inspection

 Poor safeguarding practice that leaves children at risk of harm.  Ineffective risk management strategies.  Poor care planning arrangements.  Significant shortfalls in the leadership and management of the home.  Poor arrangements for the education of children.  Lack of support and supervision for staff.

p.2

Outcome from the inspection

This assurance inspection was conducted due to the police raising concerns with Ofsted about children going missing from the home and the ability of staff to manage and respond to children’s escalating behaviour.

p.2

Outcome from the inspection

This inspection found serious and widespread concerns in the protection of children and the leadership and management of the home. Ofsted is not assured that managers and staff have a clear understanding of their safeguarding responsibilities to ensure that children remain safe from harm. As a result, a compliance notice under Regulation 12 (The protection of children standard) has been issued.

p.2

Outcome from the inspection

Since the previous inspection, one child has moved out of the home and two children have moved in. At the time of this inspection, three children were living at the home.

p.2

Outcome from the inspection

Risk management is inadequate. Despite being provided with relevant information from children’s placing authorities, managers do not ensure that staff have clear and accurate guidance to help them safely care for and support the children in their care if a child goes missing.

p.2

Outcome from the inspection

Information provided to staff about how to respond to children going missing is inaccurate and of poor quality. In addition, staff have failed to follow the guidance that has been provided. For example, failure to follow guidance has resulted in staff waiting three hours after they lost sight of children to report them missing. These shortfalls leave children at risk of significant harm.

p.2

Outcome from the inspection

Managers failed to thoroughly consider information provided at the point of referral about one child’s behaviour and the associated risks. Consequently, the assessment of the risk and impact of the child moving into the home was weak. The poor assessment of, planning for and response to risk has contributed to two children going missing from care together. On one occasion, this resulted in them being found in a different county. Another child has been subjected to emotional abuse through bullying and racial abuse. As a result, managers have decided that one child Assurance inspection report for children’s home: 2633744 3 will leave the home despite only recently moving in. The impact of this poor planning has compromised children’s experiences and safety.

p.2

Outcome from the inspection

Management arrangements are inconsistent. Since the last inspection, four managers have been in post. This has resulted in a lack of effective oversight of the operation of the home. Managers’ oversight of the care and safety of children is poor. Managers have not sufficiently reviewed incidents to assure themselves that children are being protected from harm or to identify learning to improve practice. This increases the risk of harm to children.

p.3

Outcome from the inspection

The arrangements to promote children’s education are poor. None of the children attend full-time education despite one child living at the home for four months. A lack of daily routine and structure means that opportunities for meaningful activities that enhance children’s learning have been lost.

p.3

Outcome from the inspection

Supervision of practice for staff is poor and does not provide staff with guidance and direction to support children effectively. Staff have not received supervision since August or September 2023. This is despite some members of the team being in their probationary period and requiring fortnightly supervision. These shortfalls do not provide reassurance that staff have the skills, ability and guidance to safeguard children.

p.3

Outcome from the inspection

A new management team is now in place at the home. Managers have accepted the findings of this inspection and plan to address the shortfalls. Assurance inspection report for children’s home: 2633744 4
