p.1
Information about this residential family centre
Date of previous inspection: 3 January 2023
p.1
This monitoring visit
Ofsted carried out this visit to monitor compliance notices served under regulations 8, 12, 13A and 19 following the centre’s full inspection on 3 January 2023 when it was judged as inadequate. The compliance notices related to several shortfalls in relation to the leadership and management of the centre in respect to safeguarding children and families. In addition to this, there were several shortfalls regarding the quality, evidence base and oversight of parenting assessments.
p.1
This monitoring visit
At the time of this monitoring visit, three families had moved out and there were no families living in the centre. Inspectors found that suitable action had been taken to meet three of the compliance notices. The compliance notice under regulation 12 was not met and so it has been restated. Inspection report for residential family centre - monitoring visit: Kaleidoscope Assessment Services 2 Following the inadequate judgement after the last inspection, immediate action was taken by the manager to implement hourly checks by staff and regular checks by managers. This included unannounced calls from the responsible individual to check that staff were following appropriate safeguarding procedures and keeping families safe. Staff have had additional training in relation to safeguarding and medication management following the concerns identified at the last visit.
p.1
This monitoring visit
The manager has introduced signed agreements for staff, to address the procedures for monitoring CCTV to keep children safe and the importance of reporting any serious concerns to an on-call manager and the placing authority without delay. Furthermore, a written agreement is now in place to direct staff to attend a family’s flat immediately should any safeguarding incidents arise.
p.2
This monitoring visit
The registered manager has attended additional training to revisit her understanding of safeguarding children and families. She has undertaken some reflective work specific to previous safeguarding incidents and identified lessons learned. She has also introduced an incident analysis record, to help her adopt a more robust approach to reviewing incident reports and scrutiny of incident management.
p.2
This monitoring visit
The manager has met with the staff team to reinforce her expectations of staff responses to safeguarding incidents. Two of the three incidents reviewed at the last inspection have been retrospectively reported by the manager to the local authority designated officer (LADO.) The two incidents have also been addressed formally with staff in supervision, to reflect on how the incidents could have been dealt with differently. However, for one incident, there has been no reflective practice and it has not been formally addressed with staff or reported to the LADO. This means that not all staff have been held accountable or supported to understand the serious implications of their practice.
p.2
This monitoring visit
The manager has amended impact risk assessments to consider the impact of families moving into the centre on those families already living there, including strategies to manage the risks. However, she has failed to consider the impact of families already living in the centre on new families moving in. The manager has not evidenced careful consideration of whether families can live with one another safely.
p.2
This monitoring visit
Managers’ monitoring of medication forms has been introduced, to ensure that all medication administered by a parent to their child is reviewed on a weekly basis. Additional safeguards are in place for staff, to ensure that parents administer safe and appropriate dosages of medication to their children.
p.2
This monitoring visit
An external company has been commissioned to undertake any internal investigations relating to allegations made against senior managers. This is because all members of the senior leadership team are related. Leaders and managers are satisfied that this is a robust process, as their allegation management procedures include involving the LADO and Ofsted. The manager has revisited the whistle- blowing policy in relation to these changes. Inspection report for residential family centre - monitoring visit: Kaleidoscope Assessment Services 3 Leaders and managers have made arrangements to ensure that parenting assessments undertaken by the social worker who is employed by the centre are appropriately quality assured. A qualified senior social worker has been identified to oversee the parenting capacity assessments for court and supervise the centre’s social worker to ensure that she conducts her role safely and effectively.
p.2
This monitoring visit
Parents are now provided with a 12-week assessment plan that sets out the specific sessions to be undertaken on a weekly basis with the centre’s social worker. All sessions carried out by the social worker since the last inspection have been clearly recorded. Parents’ weekly planners include additional direct work that parents undertake with staff to support and develop their parenting skills. This means that parents have structured weekly plans and time to prepare themselves for the sessions.
p.3
This monitoring visit
A pre-admission document request list and an escalation policy for any information not received have been introduced for new families moving into the centre. This is to ensure that the centre holds all the relevant documents from placing authorities to inform their assessment. Placing authority social workers and the staff team will also attend pre-placement meetings, to ensure that all vital information is shared with the centre before families move in.
p.3
This monitoring visit
The remaining areas of concern raised at the full inspection were not looked at during this visit. Therefore, they remain as requirements until the centre’s next inspection. Inspection report for residential family centre - monitoring visit: Kaleidoscope Assessment Services 4
