Inspection report for children's home - monitoring visit: 2553111 1 Registered provider: Lakeside @ Our Place Limited

p.1

Inspected under the social care common inspection framework

p.1

Information about this children’s home

A private company operates this children’s home. It provides care for 10 children with learning disabilities, including autism spectrum disorder and associated challenging behaviours.

p.1

Information about this children’s home

A new manager has been appointed. He has applied to register with Ofsted.

p.1

This monitoring visit

This home was judged requires improvement to be good at the last full inspection. On 15 November 2022, a monitoring visit was carried out following concerns raised with Ofsted about the quality of care provided to children and in response to the provider notifying Ofsted of seven medication errors since 9 September 2022.

p.1

This monitoring visit

Inspectors found serious and widespread failings regarding the administration and management of medication. As a result of the visit, Ofsted issued a notice restricting accommodation at the home and a compliance notice under regulation 23, medicines.

p.1

This monitoring visit

This latest monitoring visit took place to review the provider’s adherence with the notice of restriction of accommodation and to evaluate what steps the provider had taken to meet the compliance notice.

p.1

This monitoring visit

The inspectors found that the provider has complied with the notice restricting accommodation. Since the last inspection, no children have moved in or out of the home. There are currently nine children living in the home.

p.1

This monitoring visit

Inspectors found that since the last visit there have been no medication errors. Managers have changed the home’s medication policy. The policy now provides Inspection report for children's home - monitoring visit: 2553111 2 clearer guidance in relation to the administration of medication when children are away from the home, the monitoring systems that are used to oversee medication processes and the procedure to follow in the event of a medication error. This has supported safer administration of medication and reduced the risks to children.

p.1

This monitoring visit

The manager has investigated the medication errors that were identified at the last visit and responded to shortfalls in staff practice. Learning from previous errors has taken place. Actions following these reviews have included the implementation of a staff handover procedure, which assigns responsibility to a named medication officer. In addition, an error recording form has been created to record the circumstances surrounding any errors and to aid future investigations. The manager has also updated the local safeguarding team on the actions that have been taken. The manager now has better oversight of staff practice and has implemented learning to help prevent reoccurrence.

p.2

This monitoring visit

Leaders and managers have implemented monitoring systems to ensure sufficient oversight of children’s medication. This includes weekly and monthly audits of medication. The effectiveness of this new practice will be further considered at the next inspection.

p.2

This monitoring visit

Managers have not yet ensured that staff have completed medication training in line with the home’s medication policy. In addition, managers have failed to review children’s specific needs and establish a training plan, to ensure that staff have the correct knowledge and skills to meet children’s health needs. This increases potential risks to children as not all staff are fully equipped to meet the complex needs of children living in the home. In addition, managers have not reassessed staff competency in the safe handling and administration of medication. As a result, the compliance notice under regulation 23, medicines, has not been met.

p.2

This monitoring visit

Following this visit, the restriction of accommodation remains in place. In addition, a compliance notice under 23 has been reissued. Ofsted will continue to monitor progress and review all the remaining requirements and the compliance notice at the next visit. Inspection report for children's home - monitoring visit: 2553111 3
