Allinonecare Limited

Monitoring visit Inspected under the social care common inspection framework

p.1

Information about this children's home

This is a privately run children’s home. The home is registered to provide care and accommodation for up to three children. The provider states in their statement of purpose that they provide care for children with learning difficulties and disabilities.

p.1

Information about this children's home

The children’s home was registered with Ofsted in February 2020. The registered manager was also registered in February 2020.

p.1

Information about this children's home

Due to COVID-19 (coronavirus), at the request of the Secretary of State, we suspended all routine inspections of social care providers on 17 March 2020.

p.1

This monitoring visit

The purpose of this visit was to monitor compliance with the suspension notice and cancellation notice issued by Ofsted. An inadequate judgement was given following the full inspection on 24 and 25 May 2021. The setting was suspended from operating on 28 May 2021. A cancellation notice was issued on 11 June 2021 to cancel the setting’s registration.

p.1

This monitoring visit

The suspension and cancellation notices highlighted the following issues, which were examined during this monitoring visit:

p.1

This monitoring visit

 The poor food hygiene practices.  The poor management of a child’s health needs.  The poor medication administration arrangements.  The poor leadership and management.  Staff not having the necessary skills and experience to support, safeguard and promote the well-being of children.  The ineffective management of a safeguarding concern.  The lack of staff professional boundaries.  The ineffective behaviour management strategies. Inspection report for children's home - monitoring visit: 2547511 2  The lack of staff training.  The insufficient staffing arrangements.  Transport not being provided for a child to attend school.  The poor admission and matching arrangements.  The poor record-keeping.

p.1

This monitoring visit

During the visit, the inspector spoke to the registered manager, the responsible individual and the new deputy manager. The inspector examined documents and the fridge. The inspector also observed a demonstration of the new computer system. A discussion was held after the visit with the director of a children’s homes network.

p.2

This monitoring visit

The cancellation notice highlighted poor food hygiene practices. This related to rotting potatoes, out-of-date foods and unlabelled food in the fridge. The home now has good food hygiene arrangements. Opened items were date labelled and no expired items were in the fridge. There was also no evidence of any rotting foods.

p.2

This monitoring visit

The cancellation and suspension notices highlighted that a child with a medical condition had refused to take their medication and was not taken to hospital. This was between 7 and 9 May 2021. After a 72-hour period, this was perceived to be a ‘life threatening’ situation. However, records available at this monitoring visit demonstrate that the child was taken to hospital on 9 May 2021. This child received the appropriate medical treatment. A referral was also made to the child and adolescent mental health service to formulate a management plan in relation to the child’s ongoing issues with refusing to take their medication.

p.2

This monitoring visit

The cancellation and suspension notices detail poor medication administration records. Leaders and managers confirm that medication had been given as prescribed. However, this was not always accurately recorded. There is now an improved medication administration computerised system. The computer system will automatically deduct the dosage and highlight if the wrong dosage is given. Staff have also undertaken medication administration training.

p.2

This monitoring visit

The suspension notice highlighted that leaders and managers have failed to ensure that staff have the necessary skills and experience to support, safeguard and promote the well-being of children living in the home. Leaders and managers recognise their past mistakes and have an action plan to address the shortfalls identified and to improve the outcomes for children. The first being to replace the former staff team with competent and suitably experienced individuals.

p.2

This monitoring visit

Leaders and managers are using external expertise to improve their regulatory knowledge. They are now part of various forums focused on improving children’s experiences in residential care. Leaders and managers are undergoing further training. The new deputy manager has substantial social care experience. This individual will assist with enhancing the regulatory knowledge of the leadership and management team. The deputy manager post will strengthen the management arrangements. Inspection report for children's home - monitoring visit: 2547511 3 The cancellation and suspension notices highlight the ineffective management of safeguarding concerns. Leaders and managers used the disciplinary procedures to address poor staff practice and a staff member was dismissed for gross misconduct for the safeguarding concerns. This related to a staff member using vulgar and profane language and also hiding themselves and another staff member in the office

p.2

This monitoring visit

during an incident. During this monitoring visit, the registered manager had a discussion with their designated safeguarding person about this dismissal and it was agreed that a referral to the Disclosure and Barring service is not needed.

p.3

This monitoring visit

Prior to the issue of the suspension notice, a child had acquired the personal telephone number of a staff member. The staff member did not report this to leaders and managers. This staff member is no longer working at the home. Leaders and managers have introduced a new protocol to prevent this happening again. Staff telephone numbers will also be stored on the computer system, which children will be unable to access.

p.3

This monitoring visit

The cancellation and suspension notices highlighted ineffective behaviour management strategies. Children had poor sleeping patterns, staying in bed until the afternoon and not going to bed until the early hours of the morning. Staff were staying up late at night and were playing board games with children. Children were also allowed to sleep wherever they chose, including in each other’s rooms. There was also an issue with children smoking cannabis in the home.

p.3

This monitoring visit

Leaders and managers had endeavoured to positively engage children through the provision of art-therapy sessions. Behaviour management strategies, however, were not consistently robust. Leaders and managers acknowledge that a multi-agency approach should have been taken to address a child’s insomnia. Evidence was seen of a referral to a therapist to address emotional issues that impacted on a child’s sleeping patterns. House rules now include children not going into any other bedroom without permission.

p.3

This monitoring visit

Children understand the impact of their cannabis use. They benefitted from a referral to a specialist substance misuse service for additional support. They also received a written warning in regard to smoking cannabis within the home. The safer neighbourhood police also held discussions with children regarding keeping safe and the consequences of using cannabis. The home is also a drug- and alcohol-free zone.

p.3

This monitoring visit

The cancellation and suspension notices highlight insufficient staffing. There had been an issue with four staff members being off at the same time. The proposed staffing is sufficient to meet the needs of children. Leaders and managers are employing bank staff to cover staff shortages. Agency staff will no longer be used in order to provide good continuity of care.

p.3

This monitoring visit

The registered manager and the deputy manager will be supernumerary to provide additional support and guidance to staff. Staff have relevant experience in social care, residential care and therapeutic services.

p.3

This monitoring visit

Leaders and managers recognise that the work undertaken by staff is demanding. They aim to provide the staff team with additional support, particularly in relation to Inspection report for children's home - monitoring visit: 2547511 4 their emotional well-being. There are plans for staff supervision, team meetings and staff inductions to have a more supportive environment, with the aim being to ensure that staff receive effective support to meet children’s needs.

p.3

This monitoring visit

The cancellation and suspension notices highlight the lack of staff training. Of particular concern was the lack of training to help staff care for children with autism and attention deficit hyperactivity disorder (ADHD). Staff have now completed a wide range of training to meet children’s needs. The training includes enhancing the staff team’s awareness of autism, ADHD, safeguarding, managing challenging behaviour, internet safety, and learning disabilities.

p.4

This monitoring visit

The cancellation notice highlighted that a child did not have a taxi to take her to school. This child had a historical issue of non-school attendance. The home funded a taxi and staff were available to escort this child to school. On this occasion, the child refused to go to school and, therefore, the taxi did not wait for the child.

p.4

This monitoring visit

The cancellation notice highlighted that the home’s statement of purpose did not include information to demonstrate that the home offers respite care. A social worker made a request for a short-term placement. Leaders and managers did not see this as a respite placement.

p.4

This monitoring visit

The cancellation notice raised concerns in relation to the matching process. There are improvements in the admission procedure to ensure that children are suitably matched. The new computerised system includes a matching matrix. This clearly details the matching risks associated with accepting placements. This will inform the decision- making of leaders and managers.

p.4

This monitoring visit

The cancellation notice described record-keeping as ‘poor’ and ‘confusing’. The new computerised system will provide a structured way of recording essential information. Children do benefit from regular key-work sessions. However, these records were unavailable at the time of the full inspection.

p.4

This monitoring visit

Leaders and managers have taken the compliance notices seriously. Action has been taken to address all of the shortfalls highlighted in the suspension and cancellation notices. They have invested in a new computer system to record all the required information. Children benefit from the recruitment of a new experienced staff team.

p.4

This monitoring visit

Children will receive support from staff who have appropriate training to meet their needs. The proposed staffing is sufficient to meet children’s needs. Leaders and managers have demonstrated the capacity to improve. There is a greater understanding of the need to evidence the work being undertaken to provide a high- quality service for children.

p.4

This monitoring visit

Requirements from the full inspection are repeated in this report. These requirements were not reviewed during this monitoring visit. The deadline for meeting these requirements is 20 August 2021. Inspection report for children's home - monitoring visit: 2547511 5
