p.1
This monitoring visit
The monitoring visit was carried out following incidents in which staff carried out several physical interventions in order to control inappropriate behaviour. These incidents resulted in the young person being discharged from the home. Concerns were raised by the local authorities involved, as the reports of the incidents were inconsistent. The recordings demonstrated that the staff had struggled to cope with the behaviour of one young person placed at the home after their challenging behaviour had started to escalate. The staff had used physical interventions to control this young person‟s behaviour. After a period of several days, the young person was discharged from the home because the staff were unable to continue to manage the behaviour, which had been aggressive, violent and sexualised. The young person had not been physically harmed during these incidents, but a member of staff has alleged that they were hurt by the young person. The records of the

incident were poor and inconsistent and did not include accurate details of what had happened.
p.1
This monitoring visit
The inspector found that over the period since the previous inspection there had been some staff shortages. On several shifts there had been fewer staff working than had been agreed with placing authorities. Most children and young people at the home require the full-time supervision of two staff. On some shifts, the rota showed that the staffing complement had been one staff member short. The staff Inspection report for children‟s home - monitoring visit: SC430002 2 shortages were not relevant in relation to the physical interventions being investigated, as records show that there was a full complement of staff on those shifts.
p.1
This monitoring visit
Requirements have been made to address the staff shortages and to ensure that all staff record required information more thoroughly. It was also apparent that not all staff had read all the available information on each child, and a requirement has been made to address this as well.
p.2
This monitoring visit
No other concerns about care practices were identified during this monitoring visit.
p.2
This monitoring visit
There are ongoing investigations into the allegations made and the incidents overall. The two placing local authorities are continuing their assessments of care practices at the home.
p.2
This monitoring visit
A further inspection of the home is likely following the conclusion of these investigations.
