Recommendations

 Care must be taken to ensure that prescribed medicines are only administered to the individual for whom they are prescribed. Medicines must be administered in line with a medically approved protocol. Records must be kept of the administration of all medication, which includes occasions when prescribed medication is refused. Regulation 23 requires the registered person to ensure that they make suitable arrangements to manage, administer and dispose of any medication. These are fundamentally the same sorts of arrangements as a good parent would make, but are subject to additional safeguards. Where the home has questions or concerns about a child‟s medication, they should approach an expert such as a General Medical Practitioner, community pharmacist or designated nurse for looked-after children. („Guide to the children‟s homes regulations including the quality standards‟, page 35, paragraph 7.15) This is with particular regard to staff making accurate records and referring

particular issues to the young person‟s general medical practitioner.  The registered person is responsible for leading a team which provides high- quality care for all children living in the home. They must lead and manage the Inspection report children‟s home: SC457089 5 home in a way that delivers the ethos, outcomes and approach set out in the home‟s Statement of Purpose. („Guide to the children‟s homes regulations including the quality standards‟, page 52, paragraph 10.4) This is with regard to ensuring that all staff, including occasional staff, read amendments to important documents before they work with young people.

p.4

Overall experiences and progress of children and young people: good

The home currently has one young person in placement. The young person has benefited from a carefully managed admission to the home. Staff moved with her to help provide good continuity of care.

p.5

Overall experiences and progress of children and young people: good

Staff involve the young person in direct and meaningful discussions about her care. The young person has a good understanding of her situation and is able to influence day-to- day decisions.

p.5

Overall experiences and progress of children and young people: good

The house is well presented. It is homely and offers easy access to local amenities.

p.5

Overall experiences and progress of children and young people: good

The young person health needs are addressed well. A clear and thorough plan is in place to meet her psychological and emotional needs.

p.5

Overall experiences and progress of children and young people: good

Staff administer prescribed medication at the correct intervals. However, the recording and monitoring requires improvement. Half of the staff team has not completed training in medicine administration, although the course is booked. There are some issues with the administration of medication. For example, staff have not requested a medication review by the young person‟s general practitioner regarding the ongoing use of hay- fever medication and the use of painkilling medication.

p.5

Overall experiences and progress of children and young people: good

The young person follows an education timetable, but does not have a school placement. Her social worker is addressing this as part of the carefully managed package of support.

p.5

How well children and young people are helped and protected: good

Staff keep the young person safe and understand her risks. High staffing levels mean that behavioural difficulties are managed well, and the young person is showing signs of positive progress.

p.5

How well children and young people are helped and protected: good

The staff allow the young person appropriate liberty. The manager keeps restrictions, such as bedroom door alarms, under review to ensure that they do not infringe the young person‟s rights.

p.5

How well children and young people are helped and protected: good

In most cases, the staff record incidents well. This helps to demonstrate the progress Inspection report children‟s home: SC457089 6 that the young person is making. However, some records of physical interventions are incomplete. For example, senior staff do not always arrange meetings for the staff involved in the incident and the young person to receive debriefs. This limits the opportunity to review staff practice.

p.5

The effectiveness of leaders and managers: requires improvement to be good

The last full inspection was in 2015, when Ofsted judged the service to be at a good standard. Since then, the home has had a long period of being closed. A new manager started in October 2018 when the new young person arrived.

p.6

The effectiveness of leaders and managers: requires improvement to be good

The manager has not yet had a chance to complete a six-monthly review of the service.

p.6

The effectiveness of leaders and managers: requires improvement to be good

Half of the staff are qualified to a level 3 diploma in residential childcare and other staff are on track to achieve the award within the regulatory timescale. The new manager has started her level 5 in leadership and management, and intends to apply to Ofsted for registration. The manager has arranged specialist training for her team members, to help them meet the needs of the young person.

p.6

The effectiveness of leaders and managers: requires improvement to be good

When some staff return from leave they do not always read amendments made to important documents. This creates slippage in staff knowledge and means that they are less familiar with the latest strategies for helping to keep the young person safe.

p.6

The effectiveness of leaders and managers: requires improvement to be good

Staff receive a good level of support from the manager and the therapist. As a result, they show a high level of resilience and feel rewarded by the young person‟s progress. The manager is considering ways of establishing the baseline from which staff will track the young person‟s progress.

p.6

The effectiveness of leaders and managers: requires improvement to be good

The manager has not obtained some documents, such as the official minutes of key meetings, in a timely way.

Information about this inspection

Inspectors have looked closely at the experiences and progress of children and young people. Inspectors considered the quality of work and the differences made to the lives of children and young people. They watched how professional staff work with children and young people and each other and discussed the effectiveness of help and care provided. Wherever possible, they talked to children and young people and their families. In addition, the inspectors have tried to understand what the children‟s home knows about how well it is performing, how well it is doing and what difference it is making for the children and young people whom it is trying to help, protect and look after.
