This provision is inadequate
Leadership and management of the nursery are inadequate. Staff have a poor understanding of wider safeguarding issues and do not accurately record children's hours of attendance. Areas of the premises are unclean and unsafe. Plaster is coming away from some of the walls in the rooms. Thick layers of sharp, chipped paint hang loosely from a wall and pose a significant risk to children's safety. Toilet facilities for all children are extremely poor, as they are dirty, unhygienic and pose a high risk of cross-infection. There is a leak coming from a storage room in one of the toilet areas, which forms a puddle of water on the floor. Nappy changing arrangements are poor and staff do not follow stringent hygiene practices to promote children's good health. Sleeping arrangements in the Little Stars room are inadequate. There are large storage shelves with equipment directly above where children sleep. This area is unsafe and

unsecure, and poses an extremely significant risk of injury to children. Risk assessments are ineffective. Children's safety and the safety of the premises are inadequate. The manager has failed to identify and minimise all risks across the nursery, including the outdoor areas used by children. The quality of teaching is inconsistent. At times, staff do not provide sufficient challenge and children become disengaged in their learning. Children do not make good progress.

Inspection activities
We carried out this inspection as a result of a risk assessment, following information received about this provider. The inspector had a tour of the premises with the manager. He observed the quality of teaching during activities indoors and outdoors, and assessed the impact this had on children's learning. The inspector spoke with the manager of the setting, the deputy manager, staff and the provider at appropriate times throughout the inspection. He carried out a joint observation of a focused activity and evaluated the quality of teaching with the manager. The inspector looked at relevant documentation, including records of children's learning and development, safeguarding procedures, risk assessments and a selection of policies. The inspector held a meeting with the local authority early years adviser. He spoke to a number of parents during the inspection and took account of their views.
p.3
Effectiveness of the leadership and management is inadequate
The management team has failed in their legal duties to offer children clean, safe and hygienic premises. There are significant breaches of requirements, which mean that the setting's capacity to continually improve is weak. Self-evaluation is inaccurate and does not address areas of weakness. Safeguarding is ineffective. Children's hours of attendance are not recorded correctly and in some instances, not recorded at all. Staff have a weak understanding of all possible signs, symptoms and indicators that may raise concern about a child's welfare, particularly if they are subject to extreme views or radicalisation. On the day of the inspection, the inspector found that the toilet areas used by children were dirty and unhygienic, and floors and basins had not been cleaned. The manager has failed to have a significant leak repaired in a storage room in one of the children's toilet areas, which makes the toilet floor slippery. Sleeping arrangements in the Little Stars room are
p.4

extremely poor. Carpets are unclean and children walk across bedding and sleep mats with their shoes on. Risk assessments are inadequate. The manager has failed to identify the unsuitable and unsafe storage of equipment on shelves which are directly above where children sleep. Further risks and hazards have not been identified or minimised in the outdoor area. For example, children play among litter, stones, small pieces of slate and broken plastic containers. Some hard-surfaced areas outdoors are slippery and have not been cleaned and maintained to a good standard. Staff attend supervisory sessions and have appraisal meetings with the manager. They attend some training programmes, but the impact this has on outcomes for children and improving teaching practice is not visible.
p.4
Quality of teaching, learning and assessment requires improvement
Teaching across the nursery is variable. At times, children of all ages quickly lose interest in the activities presented to them. Staff do not provide children with developmentally appropriate activities or experiences consistently. For example, staff provide a sorting activity with counters for children in the pre-school room but do not offer an appropriate explanation. Children simply pour the counters out and do not understand what to do next. Some activities across the nursery are mundane and lack challenge. Younger children show some interest in playing musical instruments and others show sound ball skills while playing outdoors. Staff support children who have special educational needs and/or disabilities, and children who speak English as an additional language appropriately. Staff liaise with other agencies and keep parents well informed of their child's progress.
p.4
Personal development, behaviour and welfare are inadequate
Due to weaknesses in staff's knowledge of wider safeguarding issues, children's welfare is not assured. Care practices across the nursery are weak. Staff have failed to identify potential hazards in the play and learning environment that have a negative impact on children's physical and emotional well-being. Arrangements for nappy changing are extremely poor. Staff are inconsistent in hygiene routines and do not ensure effective precautions are taken to reduce the risk of cross-infection. They do not follow procedures to wash their hands after changing children and to wear the appropriate protective clothing provided. Children receive healthy, wholesome and nutritious food. They have plenty of opportunities to be physically active and engage in regular multi-skilled activities with a sports coach.
p.4
Outcomes for children require improvement
Children's progress from their initial starting points is not good enough, as the range of safety concerns impede on their overall learning experiences. Children behave well and develop secure friendships. Younger children show basic signs of self-motivation, such as while playing with dough and they enjoy playing with sand. Older children show effective physical skills and confidently throw and kick balls. Children acquire simple writing skills, preparing them for their next stage in learning and their eventual move on to school.
