This monitoring visit
On 24 August 2016 a safeguarding notification was received by Ofsted about an incident in the home that had taken place on 23 August 2016. Review of this incident raised concerns about the safeguarding arrangements for one young person and the management of the incident. An unannounced monitoring visit to the home was conducted on 26 August 2016 to follow up these concerns. The visit established how the incident was managed, whether there were any areas of practice that contributed to the incident and whether the follow-up actions by the home are likely to protect the young person from future incidents. During the visit a robust investigation into the incident was conducted. This included discussions with the young person concerned, the registered manager and the staff on duty at the time of the incident. Parents of the young person were in contact with the manager during the visit. All records pertaining to the incident and

supporting risk assessments were checked. The record of the incident and discussions with the child and the staff involved concluded that the incident was accidental and could not have been reasonably foreseen. This view is supported by the young person’s consultant psychiatrist. The manager has updated the young person’s risk assessment and requested a meeting with the placing authority to strengthen the risk assessment and agree the management of any future incidents. The home was judged as inadequate in May 2016. A further inspection in July 2016 judged the home as requiring improvement. Progress is evident in all areas, improving the overall experiences of children, the quality of care and the quality of the environment. This visit concluded that there are no regulatory shortfalls relating to the management of this incident. The home continues to be appropriately managed and remains registered with Ofsted.
