Summary of complaint
On 16 August 2011, we received a notification from an outside agency that raised concerns about whether the childminder has public liability insurance. We needed to investigate these concerns to see whether the childminder was meeting the Early Years Foundation Stage welfare requirements relating to 'suitable premises, environment and equipment'. In particular, these include a requirement that states that the provider must carry and display public liability insurance for the provision.

Summary of complaint
We carried out an unannounced visit to the setting and found the provider did not have public liability insurance in place although she has recently cared for children over eight years of age in the holidays and also before and after school. She had also cared for an early years child at the beginning of the year for a very short time.

Summary of complaint
Although it was not part of the original complaint, we found that the provider was not meeting the Early Years Foundation Stage welfare requirements relating to 'safeguarding and promoting children's welfare'. In particular, the requirement to obtain necessary information from parents in advance of a child being admitted to the provision; the requirement to obtain information about who has legal contact with the child, and who has parental responsibility for the child; and the requirement to request written parental permission to the seeking of any necessary emergency medical advice or treatment in the future. Also, we found that the provider was not meeting the Early Years Foundation Stage welfare requirements relating to 'documentation'. In particular, this includes a requirement that states that the provider must keep a daily record of the names of the children looked after on the premises and their hours of attendance. Although the provider has a
